Importance Of Compliance To Antiretroviral Treatment Among HIV + /AIDS Patients Literature Review

Abstract

Aim: The purpose of this systematic review was to summarize the latest evidence on the importance of compliance to Antiretroviral Treatment ART and emphasize the negative effects of non compliance to Antiretroviral medication among HIV + /AIDS patients.Using two different databases, researches about HIV/AIDS that was made in our country and other countries was searched. We looked for the studies which were made between 2010-1015 after using the key words “HIV + /AIDS, Antiretroviral Medication, Medication Compliance”. The other criteria that we marked was “free full texts”. After these markings, we reached 190 researches. 11 studies which were about importance of Antiretroviral medication compliance and health care outcomes were included in our systematic review.Compliance to antiretroviral medication provides viral supession, decrease the risk of resistance to antiretroviral drugs, promote health outcomes and increase the quality of life. Non compliance to ART causes increase in plasma HIV RNA levels. In different studies, it was shown that increase in medication compliance reduces the plasma HIV RNA levels. Non compliance to medication order as prescribed results in resistance to antiretroviral drugs. In different studies, it was shown that decrease in medication adherence increase the risk of drug resistance among HIV + /AIDS patients. Drug resistance reduces using different ART regimen options and increases the risk of treatment failure. Because of that, health status and life quality of these patients are affected negatively. Another positive outcome of medication adherence is decrease in health care costs.It must be explained to HIV + /AIDS patients that; importance of medication adherence, positive outcomes of high medication compliance and problems that results after non compliance to ART regimens. Moreover, the factors and conditions that affect medication adherence must be detected and eliminated.

HIV + / AIDS Hastalarında Antiretroviral Tedaviye Uyumun Önemi Literatür Çalışması

Abstract

Amaç: Bu derlemenin amacı HIV + / AIDS hastalarında kullanılan Antiretroviral Tedaviye uyumun öneminin anlatılması ve uyumsuzluk durumunda karşılaşılan olumsuz durumların sistematik şekilde ve kanıtlara dayalı olarak gösterilmesidir.Ülkemizde ve yurt dışında HIV + / AIDS alanında yapılmış olan, hakemli dergilerde basılan özgün araştırmalar iki farklı arama motorundan taranarak son 5 yıl listelenmiştir. Anahtar kelimeler olarak “HIV + / AIDS, Antiretroviral Tedavi, İlaç Uyumu” yazıldıktan sonra 2010-2015 yılları narasındaki “ücretsiz tam metinler” seçilerek tarama bu şekilde yapıldı.190 çalışmadan, tedaviye uyumun sağlık çıktıları üzerine etkileri ve önemi ile ilgili olan 11 tanesi çalışmaya dahil edildi.AIDS tedavisinde ART’ye sıkı uyum; viral süpresyonun devamlılığının sağlanmasını, ilaçlara karşı direnç gelişiminin azalmasını, sağlık düzeyinin iyileşmesini ve yaşam kalitesinin artmasını sağlar. Tedaviye uyumun azalması HIV RNA düzeylerinin artmasına sebep olur. Çeşitli çalışmalarda tedaviye uyumun artması sonucunda plazma HIV RNA düzeylerinde azalma olduğu gösterilmiştir.İlaçların istem edildiği şekilde alınmaması durumlarında ilaçlara karşı direnç gelişmektedir. İlaç uyumun azalması durumunda ilaçlara direnç gelişme riskinin arttığı da çeşitli çalışmalarla gösterilmiştir. Direnç gelişmesi tedavide kullanılabilecek ilaç rejimi seçeneklerinin azalmasına ve tedavi başarısının düşmesine sebep olmaktadır. Dolayısıyla da hastaların sağlık düzeyleri ve yaşam kaliteleri olumsuz etkilenmektedir. Tedaviye uyumun bir başka olumlu çıktısı olarak da aylık sağlık giderlerini azalması gösterilebilir.Antiretroviral Tedavi alan hastalara uyumun neden önemli olduğu, uyum oranı yüksek olduğunda hangi olumlu çıktıların elde edileceği ve ilaç tedavine istem edildiği şekilde uyulamadığı takdirde hangi sorunlarla karşılaşılacağı anlatılmalıdır. Ayrıca uyumu olumsuz etkileyebilecek faktörlerin belirlenmesi ve durumun ortadan kaldırılması gerekmektedir.
