THE CORRELATION OF MATERNAL SERUM CA-125 AND ALFA-FETOPROTEIN LEVELS WİTH FIRST TRIMESTER ABORTION

Abstract

Aim: To compare maternal serum CA125 and alfa fetoprotein AFP in first trimester abortions with ongoing pregnancies to determine the clinical importance Material and Method: A prospective randomized, case-controlled clinical trial involving 116 patients at 5-12 weeks of gestation were carried out. Patients were divided into four groups Group I consisted of 34 patients with normal clinical findings; whereas Group II consisted of 32 patients with the diagnosis of threatened abortion. There were 22 patients with the diagnosis of completed/ incompleted abortion in Group III and 28 patients with the diagnosis of missed abortion in Group IV. Venous blodd samples from antecubital vein were taken from all cases. The demographic features, obstetric and gynecologic history, transvaginal ultrasonography, maternal serum CA125 and AFP levels were evaluated. Results: There was no statistically significant difference in terms of CA-125 levels between groups p=0.214 , however. median AFP levels were statistically different between groups p=0.013 . The difference was arising from higher AFP levels of Group IV. On the other hand when groups were evaluated as cases and controls, there was no statistically significant difference p=0,632 . Conclusion: Maternal serum CA-125 and AFP levels have no significant value in the prediction of abortus.

MATERNAL SERUM CA-125 VE ALFA FETO PROTEİN DÜZEYLERİNİN BİRİNCİ TRİMESTER ABORTUSLARIYLA İLİŞKİSİ

Abstract

Amaç: Maternal serum kanser antijeni CA-125 ve alfa fetoprotein AFP düzeylerini sağlıklı gebeliklerde ve birinci trimester abortuslarda karşılaştırmak ve bu farkın abortus olgularında klinik önemini saptamaktır. Gereç ve Yöntem: Prospektifrandomize, olgu kontrollü klinik çalışmaya 5-12. gebelik haftalarında 116 olgu dahil edildi ve hastalar dört gruba ayrıldı. Grup I: 34 normal seyirli gebeden, Grup II: 32 abortus imminens olgusundan, Grup III: 22 abortus kompletus/inkompletus olgusundan ve Grup IV 28 missed abortus tanısı alan hastadan oluşmakta idi. Çalışmaya dahil edilen tüm olgulardan hastaneye başvuru tarihinde antekubital venden CA125 ve AFP ölçümleri için bilgilendirilmiş onam sonrası 5 cc periferik kan örneği alındı. Tüm hastaların demografik özellikleri obstetrik ve jinekolojik öyküsü, transvaginal ultrasonografisi, maternal serum CA125 ve AFP düzeyleri değerlendirildi. Bulgular: Gruplar arasında CA-125 düzeyleri yönünden istatistiksel olarak anlamlı farklılık bulunmadı. p=0.214 AFP ortancalarıysa gruplar arasında anlamlı farklılık göstermekteydi. p=0.013 Söz konusu fark grup IV'ün AFP düzeyinin diğer gruplara göre daha yüksek olmasından kaynaklanmaktaydı, t gruplar olgu ve kontrol grubu olarak değerlendirildiğinde AFP değerleri arasında istatistiksel olarak anlamlı fark izlenmedi p=0,632 . Sonuç: Bu çalışmada maternal serum CA-125 and AFP düzeylerinin abortusu öngörmede anlamlı bir değerinin olmadığı sonucuna vardık.
