MATERNAL BEDEN KİTLE İNDEKSİNİN DÜŞÜK DOĞUM AĞIRLIKLI BEBEKLERDE 2. EVREYE ETKİSİ

Abstract

Amaç: Amacımız maternal beden kitle indeksi (BKİ), fetal büyüklük, gebelikte kilo kazanımı ve düşük doğum ağırlıklı (DDA) bebek doğuran annelerin beden kitle indekslerinin 2. evre süresine etkisini değerlendirmektir.
Gereçler ve Yöntem: Bu vaka-kontrol çalışma Haziran- Aralık 2017 tarihleri arasında vajinal doğum yapan hastaların dosya kayıtları retrospektif olarak incelenerek yapıldı. Çalışmaya düşük doğum ağırlıklı bebek doğuran 141 hasta ile normal doğum ağırlıklı bebek doğuran 204 hasta olmak üzere toplam 345 hasta alındı. 36-42 gestasyonel hafta arası, nullipar, DDA ve normal doğum ağırlığında (NDA) tekil bebek doğuran ve 1. trimester sonuna ait boy, kilo kaydı bulunanlar çalışmaya dahil edildi. Sezaryen, çoğul gebelik, fetal anomali, epidural analjezi, maternal BKİ ne göre düşük kilolu olanlar, makrozomik bebek doğuranlar dışlandı.
Bulgular: Çalışma popülasyonunun ortalanca yaşı 26.0 (IQR:7), doğumda ortanca gestasyonel hafta 39.0 (IQR:2) idi. 2. Evre süresi normal kilolu, fazla kilolu ve obezler arasında farklı değildi (p=0,867). Doğum ağırlığına göre değerlendirildiğinde, DDA ile NDA doğumlarda 2.evrenin süresi benzerdi (34 dk’ya karşı 32 dk; p=0,223). Sadece DDA doğumlar 1. trimester sonundaki maternal BKİ göre normal, fazla kilolu, obez olarak gruplandırıldığında, 2. evre süresi gruplarda benzerdi. (p=0,486) 2. evre süresinin gebelikte alınan kilo ile anlamlı olarak uzadığı saptandı. (sırasıyla, yetersiz kilo alanlarda 20 (10-45) dk, normal kilo alanlarda 28 (10-60) dk , fazla kilo alanlarda 35 (10-60) dk) (p=0,001).
Sonuç:2. Evre süresinin 1. trimester sonundaki BKİ’den, bebeğin doğum ağırlığından etkilenmediği, buna karşılık gebelikte alınan kilodan etkilendiği sonucuna ulaşıldı.
