Alzheimer Hastalığı ve Oral Mikrobiyota

Abstract

Alzheimer hastalığı, dünya çapında milyonlarca insanı etkileyen hücre dışı Beta-amiloid plakları birikimi ve nörofibriler düğümlerde hücre içi hiperfosforile Tau proteini birikimi ile karakterize bir hastalıktır. Alzheimer hastalığının başlaması ve ilerleyişi ile ilişkili genetik ve çevresel risk faktörleri heterojen ve nadirdir. Bu faktörlerin yanı sıra artan yaş, beslenme, sigara/alkol, kronik kalp hastalığı ve fiziksel aktivite durumu da Alzheimer hastalığını etkilemektedir. Ayrıca son yıllarda yapılan çalışmalar sonucu mikrobiyotanın özellikle de oral mikrobiyota ile Alzheimer hastalığı arasında potansiyel bir bağlantı olduğu görülmektedir. Alzheimer hastalığı ve ağız sağlığı ilişkilidir, çünkü Alzheimer hastalarında tipik olarak bulunan bilişsel bozukluk ağız motor becerilerinde (yani, dudak hareketlerinde bozulma ve yutma fonksiyonunda azalma) ve ağız bakımı yapma yeteneği dâhil normal günlük aktivitelerin gerçekleştirilmesinde zorluklara yol açar ki bu da periodontitis ve çürük gelişme riskini artırır. Alzheimer hastalığı tedavisinde farmakolojik tedavinin yanı sıra risk faktörü olan/olmayan, fiziksel ve bilişsel olarak sağlıklı bireylerde yapılan diyet müdahale çalışmaları, beyindeki Beta-amiloid plakları birikimi, serebral glukoz metabolizması ve artan gri madde hacmi gibi Alzheimer hastalığı ile ilgili biyolojik ve görüntüleme parametrelerinde değişiklik yapma olasılığını ortaya koymuştur. Özellikle bu müdahaleler D ve B12 vitaminleri ve çoklu doymamış yağ asitleri bakımından zengin diyetlere uyulmasına dayanmaktadır.
