PARATİROİT NAKLİ BEKLEYEN HİPOPARATİROİDİ HASTALARININ NAKİL BEKLEME SÜRELERİNİN DEĞERLENDİRİLMESİ

Abstract

Amaç: Ülkemizde bir halk sağlığı problemi olan hipoparatiroidi hastalığının tek küratif tedavi seçeneği olan paratiroit nakli konusunda güncel farkındalığın nakil bekleme süresi açısından durumu değerlendirilmiştir. Sunulan çalışmanın amacı iatrojenik, idiyopatik veya otoimmün sebeplerle hipoparatiroidi tanısı almış ve paratiroit nakli için donör bekleme listesine başvuran hastaların, nakil bekleme sürelerinin değerlendirilmesidir. Yöntem: Değerlendirilen veriler, Aralık 2013 ve Haziran 2021 tarihleri arasında Bezmialem Vakıf Üniversitesi Hastanesine gelen ve Endokrinoloji, Nefroloji ve Genel Cerrahi polikliniklerine başvuran hastalardan retrospektif olarak toplanmıştır. Bulgular: Çalışmada değerlendirmeye alınan 319 hipoparatiroidili hastanın paratiroit nakli olabilmek için uygun bir donörü ortalama bekleme süresi 3 yıl 6 ay 12 gün olduğu gözlenmiştir. Bekleyen listesindeki bireylerin %70,9’una henüz nakil işlemi gerçekleştirilmemiştir. Hipoparatiroidi hastalarının, tedavi seçeneği olarak paratiroit nakli uygulamasından haberdar olmadan geçirdiği süre ise 5 yıl 1 ay ve 2 gün olarak hesaplanmıştır. Tartışma: Hipoparatiroidi hastaları açısından, donör bekleyerek geçirilen sürenin ilk defa bildirildiği bu çalışmada; tanı aldıktan sonra yaklaşık 8 yıl gibi bir süre beklediği gözlenmiştir. Psikososyal anlamda, bir halk sağlığı problemi olan Hipoparatiroidinin nakil süreçlerinde geçirilen süre açısından hem hastaların hem de ilgili sağlık çalışanlarının bilinçlendirilmesinin gerekliliği ortaya konulmuştur.

EVALUATION OF PARATHYROID TRANSPLANTATION WAITING DURATION OF HYPOPARATHYROIDISM PATIENTS

Abstract

Objective: Our aim is to define the current state of awareness about parathyroid transplantation, which is a serious public health problem in our country. The only curative treatment option for hypoparathyroidism is parathyroid transplantation. Our aim is to evaluate the waiting duration of hypoparathyroidism patients (due to iatrogenic, idiopathic, or autoimmune causes) for a suitable donor. Methods: The evaluated data were collected retrospectively from the patients who presented in the Endocrinology, Nephrology, and General Surgery outpatient clinics at Bezmialem Vakif University Hospital between December 2013 and June 2021. Results: Retrospective assessment of 319 hypoparathyroidism patients showed that the average donor waiting duration for parathyroid transplantation is 3 years, 6 months, and 12 days. Currently, 70.9% of the individuals on the waiting list have not been transplanted yet. The time spent by patients after being diagnosed with hypoparathyroidism and without being aware of parathyroid transplantation as a treatment option was determined as 5 years, 1 month, and 2 days. Conclusions: The duration of parathyroid transplantation waiting lists for hypoparathyroidism patients were reported for the first time. It has been observed that there is a particular waiting duration of almost 8 years after the diagnosis. It is psycho-sociologically important, for both patients and related health professionals, to raise awareness about this public health problem of the donor waiting process.
