Do we follow the recommendations of the guidelines for preoperative evaluation?

Abstract

We have read the recently publidhed research article
entitled “Evaluation of the Knowledge and Attitudes of the Primary Care Physicians
in Preoperative Evaluation” with great interest. As a result of this study, it
was seen that the majority of family physicians think that family physicians
should be responsible for preoperative evaluation. It is an indisputable fact
that family physicians, who are the foundation of the health system, should be
knowledgeable and responsible about preoperative evaluation. Because it is
inevitable that family physicians are better than branch physicians in
evaluating patients as a whole. As a result of the same study, it was suggested
that preoperative evaluation trainings should be increased to primary care
physicians. It is also a fact that specialist physicians do not fully comply
with the guidelines in preoperative evaluation. It is noteworthy that specialist
physicians perform extra tests in contrast to the recomendations of the
guidelines in clinical practice.

Preoperatif değerlendirmede kılavuzların önerilerini uyguluyor muyuz?

Abstract

Yakın zamanda derginizde yayınlanan “Preoperatif
Değerlendirme Konusunda Birinci Basamakta çalışan Hekimlerin Bilgi ve
Tutumlarının Değerlendirimesi” isimli araştırma makalesini ilgiyle okuduk. Bu
çalışma sonucunda görülmüştür ki aile hekimlerinin çoğunluğu preoperatif
değerlendirmede aile hekimlerinin sorumlu olması gerektiğini düşünmektedir.
Sağlık sisteminin temeli olan aile hekimlerinin bu konuda bilgili ve sorumlu
olmaları gerektiği tartışılmaz bir gerçektir. Çünkü aile hekimlerinin hastaları
bütün olarak değerlendirmede dal hekimlerine göre daha iyi olmaları
kaçınılmazdır. Yine aynı çalışma sonucunda öneriler arasında birinci basamak
hekimlerine preoperatif değerlendirme eğitimlerinin artırılması gerektiği
bildirilmiştir. Uzman hekimlerin de preoperatif değerlendirmede kılavuzlara
bütünüyle uymadıkları da bir gerçektir. Hekimlerin klinik uygulamada önerilenin
aksine daha fazla tetkik yaptıkları dikkat çekmektedir.
