Geriatrik nörocerrahi hastalarında anestezi: Retrospektif tek merkezli deneyimimiz

Abstract

Amaç
Dünyada ve Türkiye’de nüfusun giderek yaşlanmasına bağlı olarak nörocerrahi prosedürleri için bildirilen morbidite ve mortalite oranları büyük farklılıklar göstermektedir. Giderek artan hasta sayısında risk-fayda oranlarına ilişkin veri eksikliği zorlu klinik kararlara neden olabilir. Çalışmada 5 yıl içerisinde nörocerrahi alanında opere olan 65 yaş üstü hastaların cerrahi tanıları, komorbiditeleri, anestezi ve analjezi yönetimi, yoğun bakım ihtiyacı, yoğun bakım kalış süresi ve mortalite oranlarının retrospektif olarak incelenmesi amaçlanmıştır.
Materyal ve Metot
Büyük bir eğitim ve araştırma hastanesinde nörocerrahi alanında 2015-2020 yılları arasında operasyon geçiren 65 yaş üstü hastalar retrospektif olarak incelendi. Veriler yaş bağımsız ve yaş aralığına göre Grup A (66-79 yıl) ve Grup B (>80 yıl) olarak sınıflandırılarak cinsiyet, acil/elektif cerrahi gereksinimi, tanı, yoğun bakım ünitesi (YBÜ) gereksinimi, YBÜ ve hastanede kalış süresi, 30 günlük mortalite oranı ve ASA skoru bakımından karşılaştırıldı.
Bulgular
Analiz için toplam 1878 hasta dosyasına ulaşıldı. Yaş ortancası 72(68-77) yıl, % 73.8 elektif cerrahi, hastane kalış süresi 3 (2-6) gün, yoğun bakım ihtiyacı %24.8 hastada, mortalite %5.4 oranında gözlendi. Grup B’de hastanede kalış süresi, YBÜ ihtiyacı, acil ameliyata alınma oranı, 30 günlük mortalite oranı daha yüksekti (p<0.001). Grup A hastaları en sık torakolomber diskektomi (%23.7), Grup B hastaları ise kronik subdural (%22.9) operasyonu geçirdi. Hastaların büyük çoğunluğunda genel anestezi tercih edilmekteydi.
Sonuç
Yaş cerrahi ve anestezi için tek başına bir kontraendikasyon oluşturmaz. Ancak, yaşlı hasta grubunda nörocerrahi öncesi preoperatif ayrıntılı hazırlık yapılması, olabildiğince elektif şartlar oluşturulması önemlidir. Efektif anestezi ve analjezi yönetimi, yoğun bakım ihtiyacının ön görülebilmesi postoperatif oluşabilecek komplikasyonları en aza indirir.
