The Evaluation of the Provinces’ Efficiency in Terms of Family Medicine Services in Turkey

Abstract

Although the resources allocated for health care are limited, the cost of their services is increasing day by day. For this reason, there is a pressure on those who responsible for the management of health care services to use the available resources more efficiently. Measuring efficiency and reorganizing the resources according to scientific methods can contribute to the efforts in this direction. The aims of this study are to measure the technical efficiency of provinces in Turkey in terms of family health services and to determine the potential areas of improvement for increasing the technical efficiency. For this purpose, data envelopment analysis (DEA) was applied to nine inputs and eight output variables related to family health services of provinces. Input-oriented CCR model, which is developed by Charnes, Cooper and Rhodes, was used. According to the findings obtained from this study, it was determined that the average efficiency score of the provinces was 91.66% and 31 provinces were operated efficiently and 50 provinces inefficiently. In provinces operating inefficiently, the number of registered population to family medicine, analysis and examination expenses and current expenses of family health centers came to the fore as the most important areas to be intervened. It is thought that data-oriented periodic evaluations to be made on resources will contribute to the more efficient use of resources allocated for health systems.

Türkiye’deki İllerin Aile Hekimliği Hizmetleri Yönünden Etkinliğinin Değerlendirilmesi

Abstract

Sağlık hizmetleri için ayrılan kaynaklar sınırlı olmasına rağmen sağlık hizmetlerinin maliyeti günden güne artmaktadır. Bu nedenle sağlık hizmetlerinin yönetiminden sorumlu kişiler üzerinde eldeki kaynakların daha etkin bir şekilde kullanılması yönünde bir baskı olmaktadır. Etkinlik ölçümlerinin yapılması ve kaynakların bilimsel yöntemlere göre yeniden düzenlenmesi bu yöndeki çabalara katkı verebilmektedir. Bu çalışmanın amaçları, Türkiye’deki illerin aile hekimliği hizmetleri yönünden teknik etkinliğinin ölçülmesi ve etkinliğin arttırılması için potansiyel iyileştirme alanlarının belirlenmesidir. Bu amaçla illerin aile sağlığı hizmetleri ile ilişkili dokuz girdi ve sekiz çıktı değişkenine veri zarflama analizi (VZA) uygulanmıştır. Yöntem olarak Charnes, Cooper ve Rhodes tarafından geliştirilen girdi yönelimli CCR modeli kullanılmıştır. Elde edilen bulgulara göre illerin ortalama etkinlik skoru %91,66 olarak belirlenmiş ve 31 ilin etkin, 50 ilin ise etkinsiz olarak çalıştığı tespit edilmiştir. Etkinsiz olarak faaliyet gösteren illerde, aile hekimliğine kayıtlı kişi sayısı, tahlil ve tetkik giderleri ve aile sağlığı merkezlerinin cari giderleri müdahale edilmesi gereken en önemli alanlar olarak ön plana çıkmıştır. Kaynaklar üzerinde yapılacak olan veri odaklı periyodik değerlendirmelerin sağlık sistemleri için ayrılan kaynakların daha etkin kullanılmasına katkı sağlayacağı düşünülmektedir.
