Diferansiyel renal fonksiyon ve renal parankimal defektlerin Tc-99m DMSA ve Tc-99m MAG3 sintigrafileri ile karşılaştırılmalı değerlendirilmesi

Abstract

AMAÇ: Tc-99m DMSA sintigrafisi renal parankimal defektlerin saptanması
ve diferansiyel renal fonksiyonun hesaplanmasında güvenilir,

duyarlı ve sık tercih edilen bir yöntemdir. Biz çalıșmamızda pediatrik
yaș grubundaki renal hastalıkların değerlendirilmesinde Tc 99m
MAG3 ve Tc 99m DMSA’nın etkinliğini karșılaștırmayı amaçladık.
YÖNTEM: Tc-99m DMSA ile en az bir bölgede renal kortikal hipoaktif
alan, hipoaktif defekt ve/veya renal kortikal uptake’te azalma
olup olmadığı saptandıktan sonra Tc-99m MAG3 ile renal parankim
değerlendirmesi kalitatif ve kantitatif olarak yapıldı. Tc99m DMSA
ile yapılan kalitatif değerlendirmede 206 böbrekteki uyum/uyumsuzluk
değerlendirildi. Tc-99m MAG3 sintigrafisiyle de bașlangıç
renal ekstraksiyon fonksiyonları hesaplandı. Her iki yöntem istatistiksel
olarak değerlendirildi.
BULGULAR: Çalıșmada yaș ortalamaları 8,8±3,2 olan 61(%59) kız
ve 49(%41) erkek yer aldı. Tc-99m DMSA ile böbrek fonksiyonları-
na katkı, sol böbrekte 50,15±21,07 ve sağ böbrekte 49,85±21,08
bulundu. Tc-99m MAG3 ile sol böbreğin katkısı 51,10 ±22,00 ve
sağın katkısı 48,90±22,01 olarak hesaplandı. Her iki grup arasında
pozitif yönde güçlü ilișki bulundu (r=0,990, p=0,001). 155/206
(%75,25) uyumlu ve 51/206 (%24,75) kısmi uyumsuz hipoaktif alan/
defekt veya uptake’ te azalma saptandı.
SONUÇ: Tc-99m DMSA ve Tc-99m MAG3 ile renal fonksiyon de-
ğerlendirmesinde pozitif korelasyonlar saptadık.

Comparative evaluation of differential renal functions and renal parenchymal defects by using the Tc-99m DMSA and Tc-99m MAG3 scintigraphies

Abstract

AIM: Tc-99m DMSA scintigraphy is a reliable, sensitive and frequently
preferred method in both the detection of renal defects
and the calculation of differential renal function. In this study our
aim was to compare the effi ciency of the Tc 99m MAG3 with the
Tc 99m DMSA in the evaulation of renal parenchymal diseases of
the pediatric age group.
METHODS: Following the determination of at least one hypoactive
region, a hypoactive defect and/or a decrease in renal cortical
uptake in Tc-99m DMSA scan, the renal paranchyma was evaluated
qualitatively and quantitatively by using Tc-99m MAG3 scintigraphy.
By qualitative assessment, compliance/non-compliance
of the 206 kidneys were evaluated. The initial renal extraction functions
were calculated by using the Tc-99m MAG3 scintigraphy.
Both methods were evaluated statistically.
RESULTS: The study included 61(59%) female and 42 (41%) males
with a mean age of 8.8 ±3.2 years. The contributions of the left and
the right kidneys to the total renal function with the Tc-99m DMSA
scintigraphy were as 50.1521.07 and 49.85±21.08, respectively. The
contributions of the initial left and right renal extraction functions with
the Tc-99m MAG3 were as 51.10±22.00 and 48.90±22.01, respectively.
The two scintigraphy techniques were signifi cantly correlated
(r=0.990,p=0.001). There were 155 (75.25%) compliant kidneys and 51
(24.75%) kidneys with hypoactive regions/scars or decreased uptake.
CONCLUSION: There was a positive correlation between the
Tc-m99m DMSA and the the Tc--99m MAG3 reanl functional
assessments
