Koroner yavaş akım fenomeni ile P-dalga dispersiyonu ve QT-dispersiyonu arasındaki ilişkilerin belirlenmesi

Abstract

Koroner yavaș akım (KYA) fenomeni, anjiografide herhangibir koroner bulgu olmaksızın opak maddenin distale yavaș geçișiolarak tanımlanır. Altta yatan mekanizmalar henüz tam olarak anlașılamamıșolsa da, iskemi ve infarkt gibi hadiselere neden olabilmesiaçısından klinik öneme sahiptir. Bu çalıșmanın amacı koroneranjiografide saptanan KYA ile yüzey EKG bulgularından P dalgadispersiyonu ve QTc dispersiyonu arasındaki ilișkiyi belirlemektir. YÖNTEM: Çalıșmaya koroner anjiografi ișlemi yapılan ve koroner yavașakım tespit edilen 50 hasta ve 63 kontrol grubu alındı. Tüm katılımcılarınEKG değerlendirmelerinde en uzun ve en kısa P dalgası veQT dalgası değerleri belirlendi ve aralarındaki fark P dispersiyonu veQT dispersiyonu olarak kaydedildi. QTc süresinin hesaplanmasındaBazett formülü kullanıldı.BULGULAR: Hastaların yaș ortalamaları çalıșma grubunda47,15±6,17 ve kontrol grubunda ise 47,73±5,23’tü. KYA saptananhastalarda P dispersiyonu süresinin istatistiksel olarak anlamlı seviyedeyüksek olduğu saptandı.SONUÇ: KYA olan hastalarda bozulmuș atrial iletinin EKG belirteciolan P dispersiyonu süreleri artmaktadır.

Determination of the relationship between the coronary slow flow phenomenon, and the P wave dispersion and QT dispersion

Abstract

Coronary slow fl ow (CSF) phenomenon is the slow transmissionof the opaque solution to the distal portion of the coronary arteryin the absence of coronary lesions. Although the mechanismsresponsible for CSF are not completely understood, CSF is an importantclinical entity since it may cause myocardial ischemia andinfarction. The aim of this study is to determine the relationship betweenCSF, P wave dispersion and corrected QT (QTc) dispersion.METHODS: A total of 50 patients with angiographically provenCSF and 63 controls with normal coronary arteries were includedin the study. The longest and the shortest P waves and QT intervalswere determined in the electrocardiograms. The differencesbetween the shortest and the longest intervals were defi ned asdispersion. Bazett formula was used for the calculation of QTcdispersion.RESULTS: The mean ages of the patients were 47.50±6.17 in thestudy group and 47.73± 5.23 in the control group. The duration ofthe P wave dispersion was signifi cantly longer in patients with CSF.CONCLUSION: The P wave dispersion, a marker of disturbed atrialconduction seems to be longer in patients with coronary slow fl ow.
