İntestinal tıkanıklık oluşturmayan abdominal koza’nın BT ve MRG bulguları

Abstract

Abdominal koza; mekanik obstrüksiyona sebep olan, ince barsaklarıntotal ya da parsiyel çevrelenmesi ile karakterizedir. Aynı zamandasklerozan enkapsüle peritonit olarak da adlandırılır. Etiyolojisi vepatogenizi belli olmayan, laparotomi sırasında tanınan bir antitedir.Bunun için de preoperatif tanısı çok önemlidir. Bilgisayarlı tomografi(BT) ve Magnetik Rezonans Görüntüleme (MRG) preoperatiftanı için gerekli ipuçlarını gösterebilir. Bildiğimiz kadarıyla șimdiyekadar bu antiteyi sunan birkaç olgu sunumu vardır.Burada karın ağrısı ve abdominal kozanın tipik BT bulguları olanbir hasta sunulmuștur. BT bulguları üzerine kurulan preoperatif tanıcerrahi ile de doğrulanmıștır. Bu yazıda hastalığın BT ve MRG özelliklerive preoperatif tanısal ipuçları tartıșılmıș ve görüntülemeninönemi vurgulanmıștır.

CT and MRI findings of abdominal cocoon without intestinal obstruction

Abstract

Abdominal cocoon is characterized as a total or partial encasementof the small bowel by a thick fi brotic membrane leading to themechanical obstruction. It is also referred as the sclerosing encapsulatingperitonitis. It is a rare entity with an unclear aetiology andpathogenesis, and is usually diagnosed at the time of laparotomy.Thus the preoperative diagnosis is very important. Computerizedtomography (CT) and magnetic resonance imaging (MRI) may showthe signs needed for the preoperative diagnosis. To our knowledge,only a few cases of the entity have been reported previously.A patient having abdominal pain and the typical CT fi ndings of anabdominal cocoon was presented herein. The preoperative diagnosisbased on the CT fi ndings was confi rmed at the surgery. CTand MRI features of the disease and the preoperative diagnosticclues were discussed, and the role of imaging was emphasized inthis case report.
