Bupivakainle spinal anestezide, soğuk ya da pinpirick uyaranıyla duyusal blok düzeyi belirlenmesi

Abstract

Spinal anesteziden sonra duyusal bloğun dermotomal seviyesini belirlemek için geleneksel olarak pinprick uyaranı kullanılır. Pinprick uyaranı ağrılı ve invazif bir yöntemdir. Çalıșmada klinik pratikte rutin olarak kullanılan invazif pinprick uyarısıyla, non-invazif yöntem olan basit soğuk duyu kaybı testi ile belirlenmiș duyusal blok dermotomal seviyelerini karșılaștırmayı ladık.YÖNTEM: İnguinal herni onarımı yapılacak 160 yetișkin erkek hasta çalıșmamıza alındı. Oturur pozisyonda 3,5 mL volüm içinde 15 mg hiperbarik bupivakain ve 25 μg fentanil’in 27 G iğne ile L4-5 aralıktan enjeksiyonuyla spinal anestezi gerçekleștirildi. Spinal enjeksiyondan sonra 5. ve 20. dakikadaki blok seviyeleri değerlendirildi. Pinprick uyaranı için iğne, soğuk uyaranı için buz kalıbı kullanıldı.BULGULAR: Pinprick duyu kaybının dermatomal seviyesi ile soğuk duyu kaybının dermatomal seviyeleri arasında 5. ve 20. dakikada anlamlı fark vardı ve soğuk duyu kaybında dermotomal seviyeler anlamlı derecede daha düșüktü (p < 0,05).SONUÇ: Spinal anestezide blok seviyesini değerlendirmek için pinprick uyaranı yerine soğuk duyu kaybı yöntemini kullanılabilir. Ancak, aynı bireylerde pinprick testi ile kıyaslandığında soğuk uygulama testi daha așağı segment blokajı göstermektedir.

Assessment of block level with cold or pin-pirick stimulation during bupivacaine induced spinal anesthesia

Abstract

AIM: Pinprick stimulus is often used to determine the dermatomal
level of sensory block during spinal anesthesia. However, it is an
invasive and painful technique. In this study, in order to determine
the dermatomal level of sensory block during spinal anesthesia we
aimed to compare the cold sense loss and invasive pinprick tests.
METHODS: We included 160 male patients undergoing inguinal
hernia repair surgery in this prospective study. Spinal anesthesia
was performed in sitting position with a mixture of 3.5 mL heavy
bupivacaine and 25 μg fentanyl introduced intrathecally through
a 27 gauge needle at the L3-4 intervertabral space. The levels of
block were assessed at 5th and 20th minutes after the spinal injection.
A needle was used for pinprick test and an ice pack was used
for cold sensation test.
RESULTS: The dermatomal levels of sensory loss at 5th and 20th
minutes were signifi cantly different and the level of cold sensory
loss test was signifi cantly lower (p<0.05).
CONCLUSION: Cold sensory loss test may be an alternative to
pinprick test in the assessment of the block level during spinal
anesthesia. However, cold sensory test shows lower segmental
blocks in comparison with the pinprick test in the same individuals.
