Çocuk Hastalarda Böbrek Taşı Tedavisinde Retrograd Intrarenal Cerrahi-Mini Perkütan Nefrolitotomi Yöntemlerinin Karşılaştırılması: Tek Merkezli Retropektif Çalışma

Abstract

Giriş: Çocuk hastalarda böbrek taşı tedavisi önemli bir sorundur. Taş tedavisinde şok dalga litotripsi(SWL), mini-perkütan nefrolitotomi(mini-PNL), mikro perkütan nefrolitotomi(micro-PNL) ve son dönemlerde gelişen retrograd intrarenal cerrahi(RIRC) yöntemleri kullanılmaktadır. Yöntem: Dışkapı Yıldırım Beyazıt Eğitim ve Araştırma Hastanesi’nde 2012–2015 yılları arasında RIRC ve mini-PNL yapılan 1047 hastadan 17 yaş altı olan 27 hastanın demografik, intraoperatif, postoperatif verilerini ve komplikasyonlarını retrospektif olarak toplayarak değerlendirdik. Bulgular: Taş boyutu 1–2 cm olan taşlarda başarı RIRC için %70 iken, mini-PNL için %100 idi. Taş boyutu 2–3 cm arası olan taşlarda başarı RIRC için %50 iken mini-PNL için %100 bulundu. Her iki grupta major komplikasyon izlenmedi. Skopi süresi, operasyon süresi, taş yükü ve hastanede kalış süresi her iki grup arasında istatiksel olarak anlamlı şekilde farklıydı. Sonuç: Sonuç olarak her iki yöntem çocuklar hastalarda uygulanabilir, güvenli ve etkin yöntemlerdir

Abstract

AIM: Management of kidney stone in pediatric patients is an important
problem. Shock Wave Lithotripsy (SWL), mini-percutaneous nephrolithotomy
(mini-PNL), micro PNL and recently developing Retrograde
Intrarenal Surgery (RIRS) are the methods used in stone management.
METHODS: Among from the 1047 patients who had undergone
RIRS and mini-PNL in Dıskapı Yıldırım Beyazıt Training and Research
Hospital, 27 patients who are younger than 17 years were included
our study. The demographic, intraoperative and postoperative data
of these patients and complications were evaluated retrospectively.
RESULTS: For 1–2 cm diameter stones, the success for RIRS and
mini-PNL were %70 and %100, respectively. For 2–3 cm diameter
stones, the success for RIRS and mini-PNL were %50 and %100,
respectively. There was no major complication in both groups. Scopy
time, operation time, stone burden, hospital stay were different between
two groups. And the difference was statistically significant.
CONCLUSION: As a result RIRS and mini-PNL are effective and
safe methods and they are feasible in treatment of pediatric patients
