Protez Kalp Kapaklı Hastalarda Endotel Disfonksiyonunun Paravalvüler Kaçak Gelişimi Üzerine Olası Etkileri

Abstract

Amaç: Kapak replasmanı ameliyatlarının hemen sonrasında dikiş halkası etrafında kalan mikro defektlerden ve sütur diplerinden küçük boyutlarda paravalvüler kaçakların (PVK) olduğu ve postoperatif erken dönemde bu bölgede doku iyileşmesi sonucu bu kaçakların kaybolduğu gösterilmiştir. Endotel dokusunun yara iyileşmesindeki mitogenez, fibrozis, ve inflamasyonu içeren fizyolojik süreçlerde rol aldığı bilinmektedir. Bu çalışmada, protez kapak replasmanı sonrası postoperatif erken dönemde paravalvüler kaçak gelişen hastalarda endotel fonksiyonlarının araştırılması amaçlandı. Materyal ve Metot: Çalışmaya son 3 ay içerisinde protez kalp kapak replasmanı ameliyatı olup transözofajeyal ekokardiyografik inceleme sonucu PVK tesbit edilen 33 hasta (ortalama yaş: 47,9±10,4 yıl; 14 kadın) ile kontrol grubu olarak protez kalp kapağı replasmanı yapılmış olup PVK izlenmeyen 40 hasta (ortalama yaş: 49,9±13,0 yıl; 19 kadın) olmak üzere toplam 73 hasta dahil edildi. Hastaların endotel fonksiyonlarını reaktif hipereminin neden olduğu endotel bağımlı akım aracılı genişleme (Flow Mediated Dilation, FMD) ölçülmesi ile
değerlendirildi. Bulgular: PVK grubu ile kontrol grubu arasında demografik, ekokardiyografik parametreler ve hemoglobin ile laktat dehidrogenaz (LDH) düzeyleri hariç laboratuvar verileri açısından anlamlı bir fark izlenmedi. PVK grubunda hemoglobin düzeyleri anlamlı olrak düşük iken LDH düzeyleri anlamlı olarak yüksekti. Hesaplanan FMD değerleri PVK grubunda kontrol grubuna kıyasla anlamlı olarak daha düşük bulundu (12,9±1,8 ve 13,8±1,6; p=0,022). Çok değişkenli analizde, düşük FMD değeri ve yüksek LDH düzeyleri PVK için bağımsız öngördürücüler olarak belirlendi. Ayrıca, FMD değerleri ile paravalvüler defekt sayıları arasında zayıf da olsa anlamlı negatif korelasyon izlendi (r= -0,246; p=0,036). Sonuç: Protez kapak hastalarında gözlenebilen endotel disfonksiyonu postoperatif erken dönemde paravalvüler kaçak gelişiminde önemli rol alıyor olabilir
