Tip 2 Diyabetes Mellitus Hastalarında HbA1c ile Serum Magnezyum Düzeylerinin Karşılaştırılması

Abstract

Amaç: Çalışmada tip 2 diyabetes mellitus (T2DM) hastalarında serum magnezyum (Mg) düzeyleri ile serum glikoz regülasyonu arasındaki ilişkinin araştırılması amaçlandı.
Materyal ve Metot: Çalışma Kafkas Üniversitesi Tıp Fakültesi iç hastalıkları polikliniklerine başvuran 152 T2DM hastası ve 57 DM olmayan kişi alındı. Hasta grubu HbA1 c düzeyine göre <%6,5 ve ≥%6,5 olanlar olarak, Mg’a göre ise <1,6 ve ≥1,6 (mg/dl) olarak gruplandırıldı.
Bulgular: Mg düzeyleri ile HbA1 c düzeyleri arasında negatif yönde anlamlı korelasyon saptanmıştır (r=-0,391, r2=0,153, p<0,001). Benzer şekilde; Mg, HbA1 c, HDL, trigliserid, total Kolesterol, açlık İnsülin, açlık Glikozu ve HOMA-IR düzeyleri kontrol grubuna göre T2DM hastalarında istatiksel olarak anlamlı olarak farklı bulunmuştur (p<0,05). Ayrıca, hastaların HbA1 c <%6,5 ve HbA1 c ≥%6,5 grupları arasında Vitamin D, HDL, LDL, trigliserid, total kolesterol, kalsiyum, fosfor, açlık İnsülin düzeyleri arasında anlamlı bir fark saptanmamıştır (p>0,05). Bu gruplar arasındaki Mg (<0,001), HOMA-IR (0,039) ve açlık glikozu düzeyleri (<0,001) kıyasladığında ise fark istatistiksel olarak anlamlıdır.
Sonuç: T2DM hastalarında serum Mg seviyesi sağlıklı kontrol grubuna göre daha düşüktür. Ayrıca, glikoz regülasyonu bozuk T2DM hastalarında daha düşük Mg seviyeleri saptanmıştır. Bu bulgular, T2DM ile Mg eksikliği arasındaki kısır döngüyü kırabilmek için diyabetik hastalarda Mg düzeyi izlemenin ve tedavi stratejileri üzerine çalışmanın önemini ortaya koymaktadır.
