Yüksek Seviyeli Radial Sinir Hasarı Sonrası Tendon Transferleri: Retrospektif Çalışma

Abstract

Amaç: Yüksek radial sinir felci dirseğin proksimalinde bir yaralanma olarak tanımlanır. Üst ekstremite tendon transfer prosedürleri için en yaygın endikasyon, iyileşme potansiyeli olmayan periferik sinir yaralanmasıdır. Primer tedavi şansını yitirmiş ve travmadan çok geç dönem sonrasında tarafımıza başvurmuş yüksek seviyeli radial sinir felci olgularında uyguladığımız tendon transferlerinin sonuçları değerlendirildi.
Materyal ve Metot: Yüksek seviyeli radial sinir yaralanması nedeniyle primer tedavi şansını yitirmiş ve tendon transferi uygulanan, 2013–2019 yılları arasında başvuran ondört hasta çalışmaya dahil edildi. Hastanın dominant eli, işi, beklentisi ve sosyokültürel düzeyi göz önünde tutuldu. Değerlendirme için eklem hareket aralıkları ve kas güçleri ölçüldü. Fonksiyonel değerlendirme Tajima ölçütlerine göre yapıldı.
Bulgular: On dört hastamızın kas kuvveti değerlendirmesinde; ortalama el bileği ekstansiyonu 3,85; parmak ekstansiyonu 3,78; başparmak 3,5 gücünde bulundu. Ortalama el bileği eklem hareket aralığı 40,35°; ekstansiyon 16,78° (10°–70°), fleksiyon 23,57° (15°–50°) ölçüldü. Ortalama metakarpofalangeal eklem ekstansiyonu 1,92° (1°–10°), interfalangeal eklem ekstansiyonu 9,64° (0°–30°) ve başparmak abdüksiyonu 29,64° (30°–55°) derece idi. Tajima ölçütlerine göre fonksiyonel açıdan ortalama orta-iyi olarak değerlendirildi.
Sonuç: Primer tedavi şansını yitirmiş yüksek seviyeli radial sinir felci geç rekonstrüksiyonunda doğru planlama, uygun tendon ve sütür materyali seçimi yapılarak tendon transferleri ile başarılı ve yeterli fonksiyonel sonuçlar elde edilebilmektedir.
