Konjenital Kalp Anomalili 0-3 Aylık Bebeği Olan Annelerde Postpartum Uyum Sorunları ve Annelik Deneyimleri

Abstract

Amaç: Konjenital anomalili 0-3 aylık bebeği olan annelerde postpartum uyum sorunları ve annelik deneyimlerini belirlemektir.
Yöntem: Tanımlayıcı tipte ve Şubat 2021–Şubat 2022 tarihleri arasında, İstanbul’da bir eğitim araştırma hastanesinde yürütülen araştırmanın örneklemine, termde (>37 gebelik haftasında) konjenital anomali ile doğmuş olup konjenital kalp cerrahisi sonrası hastanede yatmakta olan 0-3 aylık bebeği bulunan ve araştırmayı kabul eden 111 anne dahil edildi. Verilerin toplanmasında anne ve bebeklerin tanıtıcı özelliklerini içeren bir anket formu, Anne Bebek Temas Engelleri Ölçeği (ABTEÖ) ve Postpartum Kendini Değerlendirme Ölçeği (PPKD) kullanıldı.
Bulgular: Anneler gebeliklerin çoğunun ilk gebelik %70,3 (n=78) ve planlı gebelik olduğunu %66,7 (n=74), çoğunluğun doğum öncesi konjenital kalp hastalıkları veya başka bir tanı almadığını %82 (n=91) ve çoğunluğun sezaryen doğum %65,8 (n=73) gerçekleştirdiğini bildirmişlerdir. Ölçeklerden alınan toplam puan incelendiğinde ABTEÖ toplam puan ortalamasının 46,60±9,79 (min.18- max. 90), PPKD toplam puan ortalamasının 128,99 ± 28,17 (min.- 82- max. 328) olduğu görülmüştür. ABTEÖ ile PPKD ölçekleri arası toplam puanlar ile pozitif yönde orta düzey (r= 0,331) ilişki bulunmuştur (p<.01).
Sonuç: Çalışmamızda konjenital kalp anomalili bebeği olan annelerin kendilerini değerlendirdiklerinde, postpartum anne bebek ilişkilerinde uyum zorluklarını ortalamanın altında yaşadıkları, fiziksel temas engelleri ile karşılaştıkları ve annelik deneyimlerini ortalamanın altında tanımlayabildikleri söylenebilir.


Postpartum Adjustment Problems and Motherhood Experiences in Mothers with 0-3 Months-Old Babies with Congenital Heart Anomaly
Abstract
Objective: To determine postpartum adjustment problems and motherhood experiences in mothers with 0-3 months old babies with congenital anomalies.
Method: The sample of the descriptive study, which was conducted in a training and research hospital in Istanbul between February 2021 and February 2022, included a 0-3 month old baby who was born at term (>37 weeks of gestation) with a congenital anomaly and was hospitalized after congenital heart surgery and accepted to participate in the study. 111 mothers were included. A questionnaire including descriptive characteristics of mothers and babies, Mother Infant Contact Barriers Scale (MICBS) and Postpartum Self Evaluation Questionnaire (PSEQ) were used to collect data.
Findings: Mothers stated that most of the pregnancies were first pregnancy 70.3% (n=78) and planned pregnancy 66.7% (n=74), the majority of them did not have congenital heart diseases or any other prenatal diagnosis 82% (n=91) and the majority of them had cesarean section. reported that 65.8% (n=73) did. When the total score obtained from the scales was examined, it was seen that the total mean score of MICBS was 46.60±9.79 (min.18- max. 90), and the mean total score of PSEQ was 128.99 ± 28.17 (min.-82-max. 328). A moderate positive (r= 0.331) correlation was found between the total scores of MICBS and PSEQ scales (p<.01).
Conclusions: In our study, it can be said that when mothers with a baby with congenital heart anomaly evaluate themselves, they experience difficulties in adjustment in postpartum mother-infant relations below the average, they encounter physical contact barriers, and they can describe their motherhood experiences below the average.
