Radikal Prostatektomi Sonrası Uzamış Drenaj: Üretral Anastomozun Batın Dreni Tarafından Perforasyonu

Abstract

We presented a case of prolonged drainage following RRP related to the perforation of vesicourethral anastamosis (VUA) by drain and the management. Sixty seven years old man underwent RRP. After the anastomosis, a surgical drain was placed. At postoperatively 2nd day, the drainage increased and 1500 ml/ day drained during 15 days. Subsequent cystography and cystoscopy revealed that the problem was the perforation of anastomosis by surgical drain. The drainage decreased after pulling out the drain 2-3 cm. It should be considered that prolonged drainage following RRP can occur related to the perforation of VUA by surgical drain

Radikal Prostatektomi Sonrası Uzamış Drenaj: Üretral Anastomozun Batın Dreni Tarafından Perforasyonu

Abstract

Kliniğimizde radikal retropubik prostatektomi (RRP) sonrası uzamış drenajı olan hastada tanı ve tedavi yaklaşımımızı aktarmayı amaçladık. Lokalize prostat adeno kanseri tanılı 67 yaşındaki hastaya RRP yapıldı. Anastomoz sonrası kaçak olmadığı kontrol edildi. Bir adet dren koyularak operasyon sonlandırıldı. Postoperatif 2. gün dreninden geleni artan ve 15 gün boyunca yaklaşık 1500ml/ gün geleni olan hastaya sistografi ve sonrasında sistoskopi yapıldığında problemin batın dreninin anastomoz hattını perfore etmesi olduğu belirlendi. Dren 2-3 cm geriye geri çekildiğinde durumun hızlı şekilde düzeldiği görüldü. RRP sonrası görülebilen uzamış drenajın, batına drenin anastomoz hattını perfore ederek içeriye girmesi sonucu oluşabileceği göz önünde bulundurulmalıdır
