Helikobakter Pylori Pozitif Ülser ve Gastritli Hastalarda SVCAM-1 Düzeyleri

Abstract

Gastrit tüm dünyada çok yaygın olarak rastlanılan bir hastalıktır. Hastalığın etyolojisinde çok çeşitli etkenler mevcuttur (toksik, viral bakteriyel, immün vb.). Farklı klinik formlarda ortaya çıkabilmektedir. Hastalığın patogenezi tam olarak anlaşılamamıştır. Bu açıdan bakıldığında özellikle en sık rastlanılan etkenlerden H. pylori tipiktir, çünkü çok değişik klinik seyir gösterebilmekte aynı zamanda lenfoma ve mide kanserine yol açabilmektedir. Adhezyon moleküllerinin inflamasyon oluşumunda önemli rol oynadıkları bilinmektedir, dolayısıyla gastrit patogenezinde de rol oynamaktadırlar. Bu çalışmada amacımız; H. pylori pozitif hastalarda SVCAM-1 düzeyinin arttığını göstermek ve inflamas-yonda diğer gastritlere göre bu yolağın daha yoğun bir şekilde kullanıldığını ortaya koymaktır. Çalışmaya önceden belirlediğimiz kriterlere uygun 49 (E:26, K:23)’u H. pylori pozitif ve 29 (E:10, K:19)’u H. pylori negatif iki grup olmak üzere toplam 78 hasta alındı. Çalışmaya katılan tüm hastalarda SVCAM-1 düzeyi ve patolojik özellikleri çalışıldı ve istatiksel olarak iki grup karşılaştırıldı; H. pylori pozitif grupta SVCAM-1 düzeyi istatiksel olarak anlamlı düzeyde yüksek bulundu. Sonuç olarak bu çalışma SVCAM-1 düzeyinin H. pylori enfeksiyonuna bağlı olarak gastrit hastalarında immunoenflamatuvar tepkide diğer etkenlerin yol açtığı gastritlere göre daha ağırlıklı bir rol oynadığını göstermektedir
