Korneal Kalınlığın Cerrahinin Uyardığı Astigmatizmaya Etkisi

Abstract

Amaç: Temporal saydam kesili korneal insizyon ve limbal gevşetici insizyon uygulanan katarakt cerrahisinde, santral kornea kalınlığının cerrahinin uyardığı astigmatizma üzerine etkisini değerlendirmek. Gereç Yöntem: Çalışmaya 33 hasta alındı. 1. grupta 1D’nin altında preoperatif astigmatizması olan kataraktlı 15 göze tek başına fakoemülsifikasyon, 2. grupta ise 1-2.5 D arasında preoperatif astigmatizması olan 18 göze bir adet limbal gevşetici insizyonla birlikte fakoemülsifikasyon uygulandı. 2. grupta postoperatif ciddi stromal ödemi olan 3 olgu hidrasyonun korneal astigmatizmayı etkilememesi için çalışmadan çıkarıldı. Kornea topografisi ve ultrasonik pakimetri, preoperatif ve postoperatif 1. gün, 1. hafta ve 1. ayda ölçüldü. Cerrahinin uyardığı astigmatizma vektör analiz programı ile hesaplandı. Santral kornea kalınlığı ve cerrahinin uyardığı astigmatizma ilişkisi lineer regresyon analizi ve korelasyon analizi ile değerlendirildi. Bulgular: Her iki grupta da santral korneal kalınlık ile cerrahinin uyardığı astigmatizma arasında negatif korelasyon bulundu. Bu korelasyon 2. grupta istatistiksel olarak anlamlıydı. Sonuç: Cerrahinin uyardığı astigmatizma ile santral korneal kalınlık arasında tespit ettiğimiz ilişkinin subakut korneal değişiklikleri değerlendirmede ve uzun vadeli sonuçların tahmininde kullanılabileceğini düşünmekteyiz
