REGIONAL DIFFERENCES IN COLORECTAL CANCER MORTALITY, TÜRKİYE

Abstract

OBJECTIVE: The aim of this study was to analyze the trends in colorectal cancer mortality for the last thirteen years in terms of geographical regions, age and gender.
MATERIAL AND METHODS: The data used in this study were obtained from the Turkish Statistical Institute mortality database. Age-standardized mortality rates were calculated using the world standard population. Joinpoint Regression Analysis was used to determine the statistical significance of differences in colorectal cancer mortality trends. For each statistically significant change point, annual percent change, average annual percent change, and their 95% confidence intervals were calculated.
RESULTS: Approximately 84,000 people died from colorectal cancer during the study period. The standardized mortality rate in Türkiye during this period was 6.6 per 100,000 population. According to the results of Joinpoint Regression Analysis, in men, statistically significant decreases were observed only in TR1 (İstanbul) region during the period, while statistically significant increases were observed in TRA (Northeastern Anatolia) and TRB (Middle East Anatolia) regions. In females, statistically significant decreases were observed in TR1 and TR4 (Eastern Marmara), while statistically significant increases were observed in TRA, TRB and TRC (Southeastern Anatolia) regions. When analyzed by age groups, significant increases were observed in the 45-64 age group in the 2009 - 2013 period and in the 75-84 age group in the 2009 - 2014 period for males, while the highest increase was observed in the 15-44 age group in the 2009 - 2014 period for females.
CONCLUSIONS: According to the results of the study, significant increases were observed in TR2 (Western Marmara) region for females, TR2 and TR4 for males in the last 4-5 years. It can be recommended to analyze the availability and performance of health services as well as risk factors.

KOLOREKTAL KANSER MORTALİTESİNDEKİ BÖLGESEL FARKLILIKLAR, TÜRKİYE

Abstract

AMAÇ: Bu çalışmanın amacı son on üç yıla ait kolorektal kanser mortalitesindeki eğilimleri coğrafi bölge, yaş ve cinsiyet bakımından incelemektir.
GEREÇ VE YÖNTEM: Çalışmada kullanılacak veriler Türkiye İstatistik Kurumu ölüm veri tabanından elde edildi. Yaşa göre standardize edilmiş mortalite oranları Dünya standart nüfusu kullanılarak hesaplandı. Kolorektal kanser mortalite trendlerindeki farklılıkların istatistiksel anlamlılığını belirlemek için Joinpoint Regresyon Analizi kullanıldı. İstatistiksel olarak anlamlı her bir değişim noktası için yıllık yüzde değişim, ortalama yıllık yüzde değişim ve bunlara ait %95 güven aralıkları hesapladı.
BULGULAR: Çalışma periyodu boyunca yaklaşık 84.000 kişi kolorektal kanserinden hayatını kaybetmiştir. Bu dönemde Türkiye’de standartlaştırılmış mortalite oranı 100.000 kişide ortalama 6,6 olarak elde edilmiştir. Joinpoint Regresyon Analizi sonuçlarına göre erkeklerde, periyod boyunca sadece TR1 (İstanbul) bölgesinde istatistiksel olarak anlamlı düşüş gözlenmiş iken TRA (Kuzeydoğu Anadolu) ve TRB (Ortadoğu Anadolu) bölgelerin de istatistiksel olarak anlamlı artışlar ortaya çıkmıştır. Kadınlarda ise periyod boyunda TR1 ve TR4 (Doğu Marmara) istatistiksel olarak anlamlı düşüş, TRA, TRB ve TRC (Güneydoğu Anadolu) bölgelerinde istatistiksel olarak anlamlı artışlar görülmüştür. Yaş gruplarına göre değerlendirildiğinde erkeklerde 2009 - 2013 döneminde 45-64, 2009 - 2014 döneminde ise 75-84 yaş gruplarında anlamlı artışlar ortaya çıkarken, kadınlarda en büyük artışın 2009-2014 döneminde 15-44 yaş grubunda olduğu gözlendi.
SONUÇ: Çalışma sonuçlara göre kadınlarda TR2 (Batı Marmara), erkeklerde TR2 ve TR4 bölgelerinde son 4-5 yıl içinde anlamlı artışlar gözlenmiştir. Sağlık hizmetlerinin kullanılabilirliği ve performansının yanı sıra risk faktörlerinin de analiz edilmesi önerilebilir.
