Abstract

Objective: Hysterectomy is the second most common major surgical procedure, after cesarean section in all over the world. It has been reported that the rate of hysterectomy application is tightly associated with age and parity, and will apply nearly 20% at 55-year-old women. We aimed to evaluate hysterectomy cases in our clinic, carried out at last four years. Material and Methods: The hysterectomy operations which performed between January 2008 and December 2011 were evaluated retrospectively. Demographic and operational datas reached from patients’ files were accessed. Results: The mean age of the 284 cases applied abdominal method, 16 applied vaginal hysterectomies, 12 applied laparoscopically assisted vaginal hysterectomy were 51±8 years, 54.2±8, and 50±4, respectively. The hysterectomy were performed in 142 cases (46%) for uterine leiomyomas, with the most common indication. 45 patients were operated on because of malignancy. Colporrhaphy operation were carried out at 42 patients, and antiincontinence were at 33, simultaneously. Duration of hospitalization were found, 5.6±2.4 days, 3.7±0.6, and 3.4±2.5, in total abdominal hysterectomy, vaginal hysterectomy and laparoscopic assisted vaginal hysterectomy group, respectively (p<0.001). Conclusions: The medical status of patients, uterine size, previous pelvic surgery, presence of genital prolapse, and experience of the surgeon, affect the procedure that will be select. The choice of surgical procedure, surgeon and the patient together to decide the methods by discussing the potential benefits and losses would be more appropriate. Bir Üniversite Hastanesindeki Histerektomi Olgularının Değerlendirilmesi

Abstract

Amaç: Histerektomi, tüm dünyada sezaryenden sonra ikinci sıklıkta uygulanan major cerrahi girişimdir. Histerektomi uygulanma oranının yaş ve parite ile sıkı ilişkili olduğu ve 55 yaşındaki bayanların neredeyse % 20’sine histerektomi uygulanacağı bidirilmektedir. Bu çalışmada son dört yılda kliniğimizde gerçekleştirilmiş histerektomi olgularının değerlendirilmesi amaçlandı. Materyal ve Metod: Ocak 2008-Aralık 2011 tarihleri arasında histerektomi uygulanan olgular, retrospektif olarak incelendi. Hasta dosyalarından ulaşılan demografik ve operasyon bilgileri değerlendirildi. Bulgular: 284 olguya abdominal, 16 olguya vajinal, 12 olguya ise laparoskopik asiste vajinal histerektomi uygulandığı görüldü. Olguların yaş ortalaması sırasıyla, 51±8 yıl, 54.2±8 yıl ve 50±4 yıl saptandı. En sık myoma uteri endikasyonu ile 142 olguya (%46) histerektomi uygulandı. 45 olgu malignite nedeniyle operasyona alındı. Eş zamanlı olarak 42 olguya kolporafi, 33 olguya anti-inkontinans operasyonu uygulandı. Hastanede kalış süreleri, total abdominal histerektomi, vajinal histerektomi ve laparoskopik asiste vajinal histerektomi grubunda sırasıyla 5.6±2.4, 3.7±0.6, 3.4±2.5 gün olarak hesaplandı (p<0.001). Sonuç: Histerektomi için hangi yaklaşımın seçileceği; olgunun medikal durumu, uterusun boyutu, geçirilmiş pelvik operasyon, genital prolapsus varlığı, cerrahın tecrübesi gibi faktörlerden etkilenmektedir. Cerrahi yöntemin seçiminde, cerrah ile hastanın, yöntemlerin olası fayda veya zararlarını tartışarak beraber karar vermesinin daha uygun olacağını düşünüyoruz
