Testis torsiyonu: Olgu sunumu

Abstract

Testis torsiyonu genitoüriner aciller arasında üst sıralarda yer alan bir patolojidir. Gecikmiş vakalarda orşiektomi gerekebilir.Altı yaşındaki olgumuzda kliniğimize başvurusundan iki gün önce hafif karın ağrısı, skrotal ağrı ve hacim artışı başlamış. Ağrının şiddetlenmesi ve şişlikte artma nedenli acile başvuran hastanın yapılan muayenesinde karında hassasiyet, sol skrotumunda şişlik ve hiperemi, sol testisi palpasyonla ağrılı ve ödemli tespit edildi. Hastanın skrotal renkli Doppler USG’de ise sol testiste renk dolumu ve akım izlenmedi. Hasta sol torsiyone testis ön tanısıyla acil operasyona alındı. Operasyon sırasında siyah renkte nekrotize testis parankimine yapılan insizyonla testisin kanlanmadığına karar verilmesi ve 48 saati aşan süre üzerine hastaya sol orşiektomi yapıldı. Çocuk yaş grubu da dahil akut batın incelemesinde testis torsiyonu akılda bulundurulmalıdır
