Travmatik Bilateral Pnömotoraks: Üç Olgu Sunumu

Abstract

Toraks travmaları, acil servise başvuran travmalar arasında önemli bir yere sahiptir. Travmaya bağlı ölümlerin yaklaşık dörtte birini oluştururlar. Her iki hemitoraksta aynı anda görülen pnömotoraks olarak tanımlanan simültane bilateral pnömotoraks nadir görülen klinik bir durumdur ve genellikle travma nedeniyle meydana gelmektedir. Stabilize edilemeyen toraks travmalı olgularda posterior-anterior (PA) akciğer grafisi normal olsa bile klinik şüphe halinde toraks bilgisayarlı tomografi (BT) ile değerlendirilmelidir. Bu tür olgularda akciğer kontüzyonları ile pnömotoraks ve hemotoraksın gösterilmesinde BT’nin rutin X-ray tekniklerine göre oldukça üstün olduğu gösterilmiştir. Biz bu yazımızda; Acil Servis hekimlerinin toraks travması olan hastalarda travmatik bilateral pnömotoraksı da göz önünde bulundurmalarını ve tanısal yaklaşımda BT’nin önemini vurgulamayı amaçladık
