Investigation of antibiotic resistance status of Escherichia coli and Klebsiella spp. isolates causing urinary tract infections: 4-year study

Abstract

Objective: The aim of this study is to guide clinicians in our region in empirical treatment and to contribute to the literature by determining the frequency antibiotic resistance of E. coli and Klebsiella spp. strains that cause urinary tract infections in urine samples sent to the microbiology laboratory from various clinics. Methods: In this study, urine cultures obtained from a total of 27,443 midstream urine samples sent from various outpatient clinics and services to the Microbiology Laboratory of a tertiary healthcare institution between January 2016 and September 2019 were evaluated retrospectively. Bacterial identification and antibiotic susceptibilities were performed by conventional methods and automatic systems. In the study, the number of samples was shown as “n” and calculated as percentage (%). SPSS 25 statistical package program was used in the evaluation of the study data, and the statistical significance level of the results was accepted as p<0.05. Results: 333 Esherichia coli and 373 Klebsiella spp isolates detected in urine samples sent to the microbiology laboratory were included in the study. It was determined that the samples were most frequently sent from polyclinics (53.1%). Extended spectrum beta lactamase enzyme positivity was detected in 17.7% of the isolates. The antibiotic to which the E.coli isolates included in the study were most resistant was ampicillin (48.4%), and no antibiotic resistance was detected against fosfomycin and was statistically significant (p <0.05). The least resistant antibiotic of Klebsiella spp. strains was amikacin (4.27%), and the most resistant antibiotic was ceftazidime (58.73%). Cystitis was the most common comorbidity. Conclusion: Increasing antibiotic resistance makes the treatment of UTIs difficult. It is thought that the study data will guide clinicians in contributing to the success of treatment by selecting the appropriate antibiotic and in preventing unnecessary or incorrect antibiotic use.
