Postmenopozal Kadınlarda Kemik Mineral Yoğunluğunun Yaşam Kalitesi Üzerine Etkisi

Abstract

Amaç: Postmenopozal dönem kadınlarda yaşam kalitesinin yordayıcısı olarak kemik mineral yoğunluğu (KMY) ve ilişkili diğer faktörlerin araştırılması amaçlanmıştır.
Yöntem: Osteoporoz polikliniğinde takipli postmenopozal dönemde, kırığı olmayan 164 kadın çalışmaya dahil edildi. Katılımcıların demografik verileri, DEXA yöntemi ile belirlenen lomber vertebra (L1-4 total), femur boyun ve femur toplam KMY değerleri ve t skorları kaydedildi. Ayrıca katılımcıların yaşam kalitesi, kısa form-36 (KF-36) ve QUALEFFO-41 ölçekleri ile değerlendirildi. Dünya Sağlık Örgütü kriterlerine göre, aynı cins erişkinlerin ortalamasına göre L1-4 vertebra, femur boyun veya femur toplam bölgelerinden en az birinde t skoru≥-1,0 SS olanlar (normal KMY) ve t skoru<-1,0 SS olanlar (düşük KMY) olarak 2 grupta incelendi.
Bulgular: Normal KMY (DEXA t skor≥-1) değerine sahip olan 48 olgu ve düşük KMY (DEXA t skor<-1) değerine sahip olan 116 olgu tespit edildi. KF-36 ölçeğinin parametreleri arasında fiziksel rol kısıtlılığı (p=0,012) ve ruhsal rol kısıtlılığı (p=0,012) parametreleri açısından istatistiksel anlamlı fark tespit edildi. QUALEFFO-41 ölçeğine ait parametrelerin tamamının KMY normal olan grupta daha düşük olduğu ancak iki grup arasındaki farkın sadece ağrı, zihinsel işlev ve toplam puan parametrelerinde istatistiksel anlamlılığa ulaştığı tespit edildi (sırasıyla p=0,021, p=0,023, p=0,024).
Sonuç: Bu çalışmada, postmenopozal dönem kadınlarda da yaşam kalitesinin azalmış olduğu gösterilmiştir. Bu durumun tek öngörücüsünün KMY olmadığı, menopoz süresinin de yaşam kalitesini olumsuz etki edebildiği saptanmıştır.

The Effect of Bone Mineral Density on Quality of Life in Postmenopausal Women

Abstract

Aim: In this study, it was aimed to investigate the effect of bone mineral density (BMD) and other related factors as predictors of quality of life in postmenopausal women.
Methods: A total of 164 postmenopausal women, without history of fracture due to osteoporosis were included in the study. BMD measurements were obtained with results of measurements from lumbar spine (L1-4), femoral neck and total proximal femur by DEXA technique. Quality of life was evaluated with SF-36 and QUALEFFO-41 scales. According to World Health Organization criteria, those with a t-score≥-1.00 SS (normal BMD) and those with a t score<-1.00 SS in at least one of the L1-4 vertebrae, femoral neck or femur total regions to the average of adults of the same sex (low BMD) were examined in 2 groups.
Results: Forty-eight cases with normal BMD (DEXA t score≥-1) and 116 cases with low BMD (DEXA t score<-1) were detected. Between two groups, statistically significant difference was found in physical and emotional role limitation parameters of SF-36 scale (p=0.012, for both parameters). It was determined that all parameters of the QUALEFFO-41 scale were lower in the normal BMD group, but difference between the two groups reached statistical significance only in the parameters of pain, mental function and total score (p=0.021, p=0.023, p=0.024, respectively).
Conclusion: It was shown that the quality of life of postmenopausal women was also reduced. It has been determined that BMD is not only predictors of this situation, and the duration of menopause can negatively affect the quality of life.
