Abstract

Frunculosis is an acute necrotic infection of the hair follicles which is caused by cougulase positive Staphylococcus aureus. We present a case of breast involvement of chronic frunculosis with typical ultrasonographic findings in order to distinguish from other lesions of the breast. A 27-year-old female patient underwent breast ultrasonography in our clinic. In the US examination; thick walled multiple cistic lesions with internal echoes and septa formations, extending subcutaneously, compatible with abcess formations were seen. The size of the largest lesion in the right breast was 24x14 mm and 9 x 6 mm in the left breast. In her past history steroid usage due to ITP was noticed. The biopsy of the beast lesions revealed acute abscess formation. Culture from the lesions revealed methicillin-sensitive Staphylococcus aureus. We should consider frunculosis in the diagnosis, when we find multiple cutaneous and subcutaneous thick-walled heterogeneous hypoechoic lesions with internal echoes and septa formations, which are compatible with abcess formation.

Memenin Fronkülosis Olgusu: Ultrasonografi Bulguları

Abstract

Fronkül, koagülaz pozitif S.aureus tarafından oluşturulan kıl köklerinin akut nekrotik bir enfeksiyonudur. Bu olgu sunumunda, meme tutulumu bulunan bir kronik fronkülozis vakasını memenin diğer lezyonlarından ayırt edebilmek için tipik ultrasonografi bulguları ile sunuyoruz. Yirmiyedi yaşında, bayan hastaya kliniğimizde meme ultrasonografi tetkiki yapıldı. Meme ultrasonografi incelemesinde en büyükleri sol memede 24 x 14 mm, sağ memede 9 x 6 mm boyutlarında olmak üzere ciltten cilt altına uzanım gösteren, internal ekolar ve septalar içeren kalın cidarlı apse ile uyumlu multiple komplike kistik lezyonlar izlendi. Hastanın öyküsünde ITP ve steroid kullanımı mevcuttu. Memedeki lezyondan yapılan biopsi sonucunda akut apse, inflamatuar süreç tanısı kondu. Kültür sonucunda metisilin sensitif S.aureus üredi. Birden fazla sayıda cilt ciltaltı yerleşimli, kalın cidarlı, internal ekolar ve septalar içeren apse ile uyumlu heterojen hipoekoik kistik lezyonlar saptadığımızda fronkülozis tanısını düşünmeliyiz.
