TALUS VE CALCANEUS’UN MORFOMETRİK ANALİZİ, TALAR VE KALKANEAL EKLEM YÜZÜ TİPLERİ

Abstract

Amaç: Bu çalışmada talus ve calcaneus’un morfometrik analizinin gerçekleştirilmesi, subtalar eklem instabilitesi, ligamentöz laksite ve artrit olguları ile ilişkilendirilen talar ve calcaneal eklem yüzlerinin morfolojik varyasyonlarının sınıflandırılması amaçlandı. Gereç ve Yöntemler: Morfometrik analiz için 72 calkaneus (40 sağ, 32 sol) ve 69 talus (37 sağ, 32 sol) üzerinde anteroposterior uzunluk, transvers genişlik, sulcus tali ve calcanei uzunluğu, genişliği ve derinliği ölçümleri, calcaneal yük kolu uzunluğu, Bohler açısı, talus’un baş-boyun uzunluğu ve boyun-gövde açısı ile eklem fasetlerin yüzey alanı ölçümü Imagej® görüntü analiz programı ile gerçekleştirildi. Talar ve calcaneal fasetler morfolojik olarak sınıflandırıldı. Elde edilen veriler istatistiksel olarak değerlendirildi, p<0,05 anlamlı kabul edildi. Bulgular: Calcaneus’un anteroposterior uzunluğu ve yük kolu uzunluğu hariç ölçülen tüm parametreleri sağ ve solda istatistiksel olarak farklı bulundu. Üç eklem yüzünün ayrı olarak bulunduğu A1 tipi calcaneus’ta (%40,3), ön ve orta eklem yüzeylerinin tam olarak kaynaştığı B2 tipi talus'ta (%69,1) en sık tespit edildi. Tüm eklem yüzlerinin kaynaşmış olarak bulunduğu C tipi calcaneus’ta %13,75'te bulunurken, talus örneklerinde saptanmadı. A1 tipi calcaneus örneklerinde, ön, orta ve arka eklem yüzey alanları sırasıyla 82,58±5,59, 83,61±7,4, 377,99±20,24 olarak bulunurken, B2 tipi talus örneklerinde, kaynaşmış ön-orta ve arka eklem yüzey alanları sırasıyla 427,1±31,47 ve 607,07±19,97 olarak bulundu. Sonuç: Sonuçlarımız radyolojik, cerrahi ve antropolojik açıdan önem arz eden talus ve calkaneus morfometrisi, talocalcaneal artikülasyon morfolojisi ve anatomik varyasyonları hakkında bilgi vermektedir.

Morphometric Analysis of Talus and Calcaneus, Talar and Calcaneal Joint Face Types

Abstract

Objective: This study aimed to conduct a morphometric analysis of the talus and calcaneus, and to classify the variations in their joint facets related to subtalar joint instability, ligamentous laxity, and arthritis. Material and Methods: For morphometric analysis anteroposterior length, transverse width, sulcus tali and calcanei length, width and depth measurements, calcaneal load arm length, Bohler angle, head-neck length and head-body angle of the talus, area measurement of the talar and calcaneal joint facets of 72 calcaneus (40 right, 32 left) and 69 talus (37 right, 32 left), were performed with Imagej® image analysis program. Talar and calcaneal joint facet patterns were classified morphologically. Obtained data were evaluated statistically, p<0.05 was considered significant. Results: A statistically significant difference was found between the right and left sides of all parameters except for calcaneal load arm length and anteroposterior length in calcaneus. In calcaneus A1 type facets (40.3%), where all three joint surfaces are found separately, and in talus B2 type facets (69.1%), where the anterior, media joint surfaces are fused, were most frequently detected. Type C; fused all facets was found in 13.75% of calcaneus and not in talus. In A1 type calcaneus, the anterior, media, and posterior articular surface areas were found to be 82.58±5.59, 83.61±7.4, 377.99±20.24, respectively, while in B2 type talus, the fused anterior-media and posterior articular surface area were 427.1±31.47 and 607.07±19.97. Conclusion: Our results provide information about the morphometry of calcaneus and talus, talocalcaneal articulation morphology and its anatomical variations which are important radiologically, surgically and anthropologically.
