Sleep Analysis of Sixth-Year Medical Students Before and After Their Emergency Medicine Internship

Abstract

Amaç: Bu çalışma, tıp eğitiminin son yılında olan intörn doktorların, acil servis stajı süresince yaşadığı uyku düzeni değişimlerini incelemeyi amaçlamaktadır.
Gereç ve Yöntemler: Prospektif olarak planlanan bu çalışma, fakültemizin 2024-2025 eğitim yılında acil tıp stajına katılan 78 intörn doktoru kapsamaktadır. Katılımcılar stajın başında ve sonunda olmak üzere iki zaman diliminde Pittsburgh Uyku Kalitesi İndeksi (PUKİ) ve gündüz uykululuk halini ölçmede Epworth Uykululuk Ölçeği (EUÖ) anketleri uygulanmıştır. Uyku düzenini etkileyebilecek psikiyatrik tanı, ilaç kullanımı veya uyku destek ürünleri kullanımı olanlar çalışma dışı bırakılmıştır. Veriler SPSS 26 programında analiz edilerek, parametrik olmayan dağılım gösteren veriler için Wilcoxon işaretli sıralar testi, gruplar arası karşılaştırmalar için Mann-Whitney U ve Kruskal-Wallis testleri kullanılmıştır. p<0,05 istatistiksel anlamlılık sınırı olarak kabul edilmiştir.
Bulgular: Katılımcıların %53,8’ini erkek bireyler oluşturmuştur. Acil tıp stajı sonrası tüm intörn doktorların hem PUKİ hem de EUÖ skorlarında staj öncesine göre anlamlı artışlar saptandı (PUKİ için p<0,001; EUÖ için p=0,003). Kadın intörnlerde staj sonrası uyku kalitesi bozulmaları erkeklere kıyasla daha belirgin bulunmuştur (PUKİ p=0,023; EUÖ p=0,024). Yaşam biçimi açısından aileyle yaşayan bireylerde, yalnız yaşayanlara kıyasla daha yüksek uyku bozulmaları saptanmıştır (PUKİ p=0,017; EUÖ p=0,018). Sigara ve alkol kullanımının uyku parametreleri üzerinde genel analizde istatistiksel olarak anlamlı bir etkisi bulunmamaktadır. Kafein tüketim miktarı ile PUKİ ve EUÖ skorlarındaki artışlar anlamlı olarak tespit edilmiştir. PUKİ’nin gündüz işlev bozukluğu alt bileşeninde staj öncesi ve sonrası değerler arasında belirgin bir bozulma mevcuttu (p=0,002).
Sonuç: Acil servis stajı, intörn doktorların uyku kalitesi ve gündüz uyanıklık düzeyleri üzerinde olumsuz etkiler oluşturmaktadır. Gözlenen bu etkiler hem bireysel sağlık hem de klinik karar verme süreçleri açısından önem arz etmektedir. Eğitim kurumlarının, staj dönemlerindeki uyku hijyeni bozulmalarını dikkate alarak intörn hekimlere yönelik uyku sağlığını destekleyici programlar geliştirmesi önerilmektedir.

Abstract

Objective: This study aims to examine the changes in sleep patterns experienced by intern doctors in their final year of medical training during their emergency department internship and to evaluate these changes using the Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (ESS).
Material and Methods: This prospective study included 78 intern doctors who participated in the emergency medicine internship at our Faculty during the 2024-2025 academic year. Participants completed the PSQI and ESS questionnaires at two time points: at the beginning and end of the internship. Participants with psychiatric diagnoses, medication use, or use of sleep aids that could affect sleep patterns were excluded from the study. Data were analyzed using SPSS 26 software, with the Wilcoxon signed-rank test used for non-parametric distributions, and the Mann-Whitney U and Kruskal-Wallis tests used for intergroup comparisons. A p-value of <0.05 was considered statistically significant.
Results: Male participants comprised 53.8% of the study population. Significant increases were observed in both PSQI and ESS scores following the emergency medicine rotation (p<0.001 for PSQI; p=0.003 for ESS). When analyzed by gender, female interns showed more pronounced deterioration in sleep quality compared to males after the rotation (PSQI p=0.023; ESS p=0.024). Regarding lifestyle factors, individuals living with family demonstrated higher sleep disturbances compared to those living alone (PSQI p=0.017; ESS p=0.018). In the general analysis, there was no statistically significant effect of cigarette and alcohol use on sleep parameters. An increase in PSQI and ESS scores was found to be significantly associated with caffeine consumption. There was a significant deterioration in the daytime dysfunction subcomponent of PSQI between pre- and post-internship values (p=0.002).
Conclusion: The emergency department rotation was associated with negative effects on intern doctors' sleep quality and daytime alertness levels. These observed effects are significant for both individual health and clinical decision-making processes. It is recommended that educational institutions develop sleep health support programs, taking into account sleep hygiene deterioration during rotation periods.
