Abstract

This letter provides a critical appraisal of the article titled “Assessing the Prognostic Value of the Hemoglobin-to-Red Cell Distribution Width Ratio in Emergency Department Patients with Acute Coronary Syndrome” by Kudu and Danış. The study explores the prognostic utility of the hemoglobin-to-RDW ratio (HRR) in acute coronary syndrome (ACS) patients admitted to the emergency department. While the investigation highlights a clinically accessible biomarker, key methodological limitations are identified. These include the retrospective, single-center design; absence of multivariate statistical adjustment for confounders; and lack of subgroup analyses based on ACS types. Additionally, the clinical applicability of HRR is questioned due to its low specificity and limited integration into established risk models. This letter underscores the need for more comprehensive, prospective studies to clarify HRR’s independent prognostic value and its role in acute cardiovascular care.

Abstract

Bu mektup, Kudu ve Danış tarafından yazılan “Acil Servisteki Akut Koroner Sendromlu Hastalarda Hemoglobin-Kırmızı Hücre Dağılım Genişliği Oranının Prognostik Değerinin Değerlendirilmesi” başlıklı makalenin eleştirel bir değerlendirmesini sunmaktadır. Çalışma, acil servise başvuran akut koroner sendrom (AKS) hastalarında hemoglobin/eritrosit dağılım genişliği oranının (HRR) prognostik değerini araştırmaktadır. Kolay erişilebilir bir biyobelirteç sunması açısından dikkat çekici olsa da, önemli metodolojik sınırlılıklar mevcuttur. Bu sınırlılıklar arasında retrospektif ve tek merkezli tasarım, karıştırıcı değişkenler için çok değişkenli istatistiksel analiz eksikliği ve AKS alt tiplerine göre alt grup analizlerinin yapılmaması yer almaktadır. Ayrıca, HRR'nin düşük özgüllüğü ve mevcut risk modelleriyle entegrasyonunun yapılmamış olması, klinik uygulamadaki kullanımını sorgulatmaktadır. Bu mektup, HRR'nin bağımsız prognostik değeri ve akut kardiyovasküler bakım sürecindeki yeri konusunda daha kapsamlı ve prospektif çalışmalara duyulan ihtiyacı vurgulamaktadır.
