Comparison of in-hospital and in Emergency Services Cardiopulmonary Resuscitation

Abstract

Introduction: The aim of this study was to compare cardiopulmonary resuscitation outcomes after the activation of Code Blue in the hospital and in the emergency services.

Material and Methods: This study was conducted retrospectively between 1 January 2011 and 30 April 2013. The results were taken into account regarding the patients-who was in Code Blue state or developed cardiac arrest in emergency room- age, gender, cardiopulmonary resuscitation (CPR) duration, results of the cardiopulmonary resuscitation and the medicatiton used through cardiopulmonary resuscitation. Also, the service or the unit of which the patients were in, the time taken by the Code Blue Team to access the patient and the leader of the team’s expertise were enlisted.

Results: The present study consists of 250 Code Blue cases and 311 emergency room (ER) cases. The patients that developed cardiac arrest in the emergency room were younger than Code Blue patients (p<0.05) but there was no difference regarding gender (p>0.05). Time taken to reach the patients and CPR duration were shorter in emergency department group and having response to CPR was higher in emergency department group. The arrest was mostly caused by asystole however, ventricular fibrillation was higher in the ER (p<0.05). Arrest patients that were Blue Coded had high infection rate (24.8%). Code Blue was mostly called by pulmonologists (24.4%) and internal medicine (20%), the call was most often responded by the same clinics as well. It was clear that age, time taken to reach the patient, cardiopulmonary resuscitation duration, medication used and ECG rhythm were factors on mortality.

Conclusion: We believe that CPR success rates in emergency department higher than Code Blue group because of more experienced staff, quick access and more efficient intervention rooms. For more effective implementation of the Code Blue, number of experienced staff should be increased and acessing to patient should be more easy.

HASTANE İÇİ VE ACİL SERVİS KARDİYOPULMONER RESÜSİTASYONLARININ KARŞILAŞTIRLMASI

Abstract

Giriş: Bu çalışmada, hastane içinde aktive edilen mavi kod sonrası uygulanan kardiyopulmoner resüsitasyon ve Acil servislerde uygulanan kardiyopulmoner resüsitasyon karşılaştırarak, mavi kod uygulamasındaki yeterlilik durumunu değerlendirmeyi amaçladık.

Gereç ve Yöntem: 1 Ocak 2011 ile 30 Nisan 2013 tarihleri arasında retrospektif olarak yapılan çalışmamızda, mavi kod verilen ve Acil serviste arrest gelişen hastaların yaş, cinsiyet, kardiyopulmoner resüsitasyon süresi, sonucu ve kardiyopulmoner resüsitasyonda kullanılan ilaçlar değerlendirildi. Ayrıca mavi kod verilen hastaların bulunduğu servis veya ünite, mavi kod ekibinin olay yerine ulaşma süresi ve mavi kod ekip lideri olan hekimin uzmanlık branşı kaydedildi.

Bulgular: Çalışmamıza mavi kod çağrısı yapılan 250 arrest olgusu ve Acil serviste müdahale edilen 331 arrest olgusu incelendi. Acil serviste arrest gelişen hastalar, mavi kod verilen hastalardan daha genç olup (p<0.05), gruplar arasında cinsiyet yönden anlamlı farklılık yoktu (p>0.05). Hastaya ulaşma zamanı ve kardiyopulmoner resüsitasyon yapma süresi daha kısa, kardiyopulmoner resüsitasyona yanıt alma oranı daha yüksekti (p<0.05). Her iki grupta da belirlenen en sık arrest sebebi asistoli iken, Acil serviste Ventriküler fibrilasyon oranının daha yüksek olduğu saptandı (p<0.05). Mavi kod verilen arrest hastalarında enfeksiyonlar (%24.8) ilk sıradaydı. En sık mavi kod çağrısının Göğüs hastalıkları (%24.4) ve Dahiliye (%20) kliniklerince yapıldığı ve yine aynı klinik uzmanlarının çağrıya en sık yanıt verdikleri belirlendi. Mortalite üzerine yaş, hastaya ulaşım süresi, kardiyopulmoner resüsitasyon süresi, kullanılan ilaçlar, EKG ritmi ve hastaların tanılarının etkili olduğu belirlendi.

Sonuç: Acil servislerde yapılan kardiyopulmoner resüsitasyon başarı oranının hastane içi mavi kodla aktive olan kardiyopulmoner resüsitasyon başarı oranından daha yüksek olmasının Acil serviste daha deneyimli personel varlığı, hızlı erişim olanağı işle etkin ve yeterli müdahale odasının varlığına bağlı olduğunu düşünüyoruz. Mavi kod uygulamasının daha etkili olması için deneyimli personel sayısının arttırılması ve ulaşımın kolaylaştırılması gerektiğini düşünüyoruz.
