NÖTROFİL VE LENFOSİT DEĞERLERİNİN AKUT KOLESİSTİT TANISINDAKİ YERİ

Abstract

Amaç: Bu çalışmada kolesistektomi geçirecek hastaların preoperative akut kolesistit tanısında nötrofil/lenfosit oranının prediktif değerinin olup olmadığının ortaya konulması amaçlanmıştır.

Gereç ve Yöntemler: Araştırma kapsamında 2018-2020 yılları arasında akut kolesistektomi ve elektif kolesistektomi yapılan kura ile seçilen toplam 100 hasta değerlendirildi. Hastaların dosyalarından yaş, cinsiyet, operasyon yöntemi (laparoskopik veya açık), yatış süresi, operasyon öncesi son bakılan nötrofil ve lenfosit sayıları kaydedildi. Histopatolojik olarak akut kolesistit ve kolelitiyazis tanısının doğrulandığı hastalar nötrofil lenfosit sayılarının oranları açısından karşılaştırılarak bu verilerin tanısal değeri ROC eğrileri ve çapraz tablolar ile belirlendi. p<0.05 değeri istatistiksel olarak anlamlı kabul edildi.

Bulgular: Akut kolesistektomi grubunda yer alan hastaların Nötrofil/lenfosit oranının elektif kolesistektomi grubunda yer alan hastalara göre istatistiksel olarak daha anlamlı yüksek olduğu saptandı (p=0.001). Lökosit ve platelet/lenfosit oranının da benzer şekilde akut kolesistektomi yapılan hastalarda elektif kolesistektomi yapılan hastalara göre belirgin olarak daha yüksek düzeyde olduğu saptandı (p=0.001).

Sonuç: Çalışmamızdan elde ettiğimiz veriler doğrultusunda nötrofil/lenfosit oranının akut kolesistit vakalarında prediktif değerinin olduğu görülmektedir.

The Role of Neutrophil and Lymphocyte Values in the Diagnosis of Acute Cholecystitis

Abstract

Objective: In this study, it was aimed to reveal whether the preoperative neutrophil-lymphocyte ratio values of the patient groups who underwent acute and elective cholecystectomy have predictive value.

Material and Methods: Within the scope of the study, patients who underwent cholecystectomy between 2018-2020 were evaluated. Age, gender, operation method (laparoscopic or open), length of stay, neutrophil and lymphocyte counts were recorded from the patients' files. Patients whose diagnosis of acute cholecystitis and cholelithiasis were confirmed histopathologically, were compared in terms of the ratio of neutrophil lymphocyte counts, and the diagnostic value of these data was determined by ROC curves and cross tables.

Results: The leukocyte and platelet lymphocyte ratio values of the patients in the acute cholecystectomy group were found to be significantly higher than the patients in the elective cholecystectomy group (p=0.001). Similarly, the neutrophil-lymphocyte ratio was found to be significantly higher in patients who underwent acute cholecystectomy compared to patients that underwent elective cholecystectomy (p=0.001).

Conclusion: In line with the data, we obtained from our study, it is seen that the neutrophil-lymphocyte ratio value is a predictive value in cases of acute cholecystitis.
