ANKİLOZAN SPONDİLİTLİ GEBEDE ANESTEZİ YÖNETİMİ

Abstract

Ankilozan spondilit seronegatif spondiloartropatiler grubundan ağrılı, kronik, inflamatuar bir hastalıktır. Ankilozan spondilit’li hastalarda zor entübasyon ihtimali, eşlik eden hastalıkların varlığı ve nöroaksial blok uygulama zorluklarını hatırlamak çok önemlidir.Biz bu olgu sunumunda 32 yaşında, 165 cm boyunda ve 96 kg ağırlığında 40 haftalık gebe hastayı inceledik. Ağız açıklığı 4 cm, tiromental mesafe 7 cm ve mallampati skoru 2 idi. Hastanın 4 yıldır ankilozan spondilit tanısı mevcuttu. Hastanın boyun ekstansiyonunda ve lomber bölge fleksiyonunda kısıtlılık tespit edildi. Gebelik esnasında gestasyonel diyabet tanısı ile takip edildiğini öğrenildi.      Hastaya oturur pozisyonda tek seferde sağ paramedian yaklaşımla spinal anestezi uygulandı. İşlem sırasında ve sonrasında herhangi bir komplikasyon olmadı.
