Abstract

Ankilozan spondilit seronegatif spondiloartropatiler
grubundan ağrılı, kronik, inflamatuar bir hastalıktır. Ankilozan spondilit’li
hastalarda zor entübasyon ihtimali, eşlik eden
hastalıkların varlığı ve nöroaksial blok uygulama zorluklarını hatırlamak çok
önemlidir.

Biz bu olgu sunumunda 32 yaşında, 165 cm boyunda ve 96 kg
ağırlığında 40 haftalık gebe hastayı inceledik. Ağız açıklığı 4 cm, tiromental
mesafe 7 cm ve mallampati skoru 2 idi. Hastanın 4 yıldır ankilozan spondilit tanısı mevcuttu. Hastanın boyun ekstansiyonunda ve lomber bölge fleksiyonunda
kısıtlılık tespit edildi. Gebelik esnasında gestasyonel diyabet tanısı
ile takip edildiğini öğrenildi.      Hastaya
oturur pozisyonda tek seferde sağ paramedian yaklaşımla spinal anestezi
uygulandı. İşlem sırasında ve sonrasında herhangi bir komplikasyon olmadı.

The Management of Anaesthesia in a Pregnant with Ankylosing Spondylitis

Abstract

Ankylosing
spondylitis is a painful, chronic, inflammatory disease in the group of
seronegative spondyloarthropathies. It is very important to remember the
possibility of difficult intubation, the presence of concomitant diseases and
neuraxial block implementation challenges in AS patients.

In this
article we presented a case report 32-year-old, 165 cm tall, 96 kg, 40 weeks pregnant
patient. The mouth opening was 4 cm, mallampati score was II and the
thyromental distance was 7cm. The patient
was diagnosed as ankylosing spondylitis for 4 years. Limitations were identified in the patient's neck extension
and flexion of the lumbar region. It is learned that she was being followed
with diagnosis of gestational diabetes during pregnancy.

Spinal anesthesia was administered to the patient in seated
position with right paramedian approach at once. There were no complications during and after the procedure.
