Comparison of CD4, CD8, and CD20 expression in sarcoid-like reactions and sarcoidosis granulomas

Abstract

Aim: Suspicious lymph node involvement on radiologic examination is a condition that requires clarified during cancer screening procedures. Sarcoid-like reaction (SLR) is a cancer mimic which is often misinterpreted as lymph node metastasis and confused with sarcoidosis. This study aimed to investigate the expression of CD4, CD8 and CD20 by immunohistochemistry in sarcoidosis and SLR granulomas in order analyze any potential difference that could be utilized in pathology practice. Method: Cases were retrospectively retrieved from the archives of Gaziantep University Faculty of Medicine Department of Pathology including 39 sarcoidosis and 27 SLR patients. Immunohistochemical staining for CD4, CD8 and CD20 was performed on representative sections. Clinical data of all cases were recorded and expression was evaluated by two pathologists both within and outside the granulomas using a 2-tier scoring system. Results: We observed similar expression patterns of CD4, CD8 and CD20 both inside and outside granulomas of sarcoidosis and SLR, thus statistical comparison wasn’t significant. Conclusion: Our study couldn’t demonstrate a difference in the immune profiles between sarcoidosis and SLR. Instead, the results suggest a similarity in immune expression, in addition to their morphological resemblance.

Sarkoid benzeri reaksiyon ve sarkoidoz granülomlarında CD4, CD8, CD20 ekspresyonunun karşılaştırılması

Abstract

Amaç: Radyolojik görüntülemelerde saptanan şüpheli lenf nodu tutulumu, kanser taramalarında açıklığa kavuşturulması gereken bir durumdur. Sarkoid benzeri reaksiyon (SBR) bir kanser taklitçisi olup, sıklıkla malign lenf nodu tutulumu olarak yanlış yorumlanmakla kalmaz, aynı zamanda sarkoidozis ile de karışır. Bu çalışmada sarkoidozis ve SBR granülomlarında immünohistokimyasal boyama yöntemleri ile CD4, CD8 ve CD20 ekspresyonlarının olası farklılıkları araştırıldı. Yöntem: Olgular retrospektif olarak Gaziantep Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı arşivinden 39 sarkoidozis ve 27 SBR hastası dahil olmak üzere toplandı. Sarkoidozis ve SBR granülomlarını içeren kesitlere immünohistokimyasal CD4, CD8 ve CD20 antikorları uygulandı. Tüm olguların klinik verileri kaydedildi ve ekspresyonlar iki patolog tarafından, granülomların içinde ve dışında olmak üzere iki kademeli bir skorlama sistemine göre değerlendirildi. Bulgular: Sarkoidozis ve SBR granülomlarının hem iç hemde dış kısmında CD4, CD8 ve CD20 ile oldukça benzer ekspresyon sonuçları gözlemledik, bu nedenle istatistiksel karşılaştırmada anlamlı sonuç saptanmadı. Sonuç: Çalışmamızda sarkoidozis ve SBR arasında immün profilleri bakımından farklılık gözlenmedi. Bilakis çalışmamız, her iki antitede görülen granülomların oldukça benzer morfolojik görüntüleri yanı sıra immün profilleri bakımından da oldukça benzer olduklarını gösterdi.
