Abstract Detection of Colistin Susceptibility in Multidrug Resistant Gram Negative Bacteria Aim: In this study, the detection of colistin sensitivity in multi antibiotic resistant strains of 121 Acinetobacter baumannii and 43 Pseudomonas aeruginosa isolates were investigated from clinical specimens, which was taken from inpatients of Mersin University, Faculty of Medicine, Health Research and Applications Services and sent to microbiology laboratory for routine diagnosis between December 2008 and August 2010. Materials and Methods: Conventional bacteriological methods and fully automated detection system (Vitek-2, BioMerieux, Fransa) were used to identify the isolates. The colistin sensitivity of the isolates was detected by Kirby Bauer disc diffusion and E-test methods according to the standards described by The Clinical and Laboratory Standards Institute (CLSI). Genotypic similarities between multiple antibiotic-resistant 164 clinical isolates and 5 environmental isolates were detected with “Arbitrarily Primed Polymerase Chain Reaction” (AP-PCR). Results: All of the A. baumannii isolates, [121 (100%)] were sensitive to colistin by E-test, 113 (93.4 %) were sensitive and, 8 (6.6 %) were intermediate sensitive to colistin by disk diffusion method. Four (100%) of the 43 P. aeruginosa isolates were sensitive to colistin by disk diffusion method, 2 (4.7%) were intermediate sensitive and 41 (95.3%) were sensitive to colistin by E-test method. According to the Gel Compar software evaluation of AP-PCR results of multiple antibiotics resistant A. baumannii strains those are isolated from inpatients; genetic proximity is found to be about 80% and it is anticipated that this can be cause of nosocomial infections. Conclusion: A very high rate of antimicrobial resistance of the isolated original strains is an indication that infections caused by those original strains became a serious problem at our hospital. High in-vitro sensitivity of original strains included in this study at the existence of such antimicrobial resistant period is evaluated to be promising for treatment options.
Türkçe Özet:Amaç: Bu çalışmada, Mersin Üniversitesi Tıp Fakültesi Sağlık Araştırma ve Uygulama Merkezi servislerinde yatan hastalardan, Aralık 2008-Ağustos 2010 tarihleri arasında Mikrobiyoloji laboratuvarına rutin tanı amacı ile gönderilen klinik örneklerden izole edilen, çoklu antibiyotik dirençli 121 Acinetobacter baumannii ve 43 Pseudomonas aeruginosa suşu kolistin duyarlılığı açısından incelenmiştir. Gereç-Yöntem: İzolatların tanımlanmasında konvansiyonel bakteriyolojik yöntemler ve tam otomatik tanımlama sistemi (Vitek 2, BioMerieux, Fransa) kullanılmıştır. Suşların kolistin duyarlılığı E-test yöntemiyle tespit edilmiştir. Çoklu antibiyotik dirençli 164 köken ile çevre örneklerinden izole edilen 5 kökenin genotipik benzerliklerini saptamak için “Arbitrarily Primed Polymerase Chain Reaction” (AP-PZR) yöntemi uygulanmıştır. Bulgular: Çalışmaya dahil edilen 121 A. baumannii kökeninin 121 (%100)\'i kolistine duyarlı, P. aeruginosa kökenlerinin ise 2\'si orta duyarlı (%4.7), 41\'i duyarlı (%95.3) olarak tespit edilmiştir. Yatan hastalardan izole edilen çoklu antibiyotik dirençli A. baumannii suşlarının AP-PZR sonuçlarının, Gel Compar yazılımı kullanılarak yapılan değerlendirme sonucunda, %80\'inin üzerinde genetik yakınlık bulunmuş ve hastane infeksiyon etkeni olabileceği düşünülmüştür. Sonuç: İzole edilen kökenlerde oldukça yüksek oranda antimikrobiyal direnç, hastanemizde bu kökenlerin neden olduğu infeksiyonların önemli bir sorun haline geldiğinin bir göstergesidir. Antimikrobiyallere direncin yüksek olduğu bu dönemde çalışmaya dahil edilen kökenlerin in vitro kolistin duyarlılığının yüksek olması tedavi seçeneğimiz açısından umut verici olarak değerlendirilmiştir.

MEU Journal of Health Sciences Assoc was began to the publishing process in 2008 under the supervision of Assoc. Prof. Gönül Aslan, Editor-in-Chief, and affiliated to Mersin University Institute of Health Sciences. In March 2015, Prof. Dr. Caferi Tayyar Şaşmaz undertook the Editor-in Chief position and since then he has been in charge.
Publishing in three issues per year (April - August - December), it is a multisectoral refereed scientific journal. In addition to research articles, scientific articles such as reviews, case reports and letters to the editor are published in the journal. Our journal, which has been published via e-mail since its inception, has been published both online and in print. Following the Participation Agreement signed with TÜBİTAK-ULAKBİM Dergi Park in April 2015, it has started to accept and evaluate online publications.
