Agonist/antagonist kontrollü over hiperstimülasyon rejimlerinde serum ve folliküler sıvı iskemi modifiye albümin düzeyleri ve gebelik oranları ile ilişkisi

Abstract

Amaç: İn vitro fertilizasyon (IVF) sikluslarında uzun GnRH analog protokol veya kısa GnRH antagonist protokol alan normoresponder hastalarda klinik gebelik oranları ve siklus parametrelerini bir oksidatif stres belirteci olan iskemi modifiye albümin düzeyleri (İMA) ile karşılaştırmayı amaçladık. Yöntem: Uzun GnRH analog protokolü alan 30 hasta ve kısa GnRH antagonist protokolü alan 30 hasta, toplam 60 normoresponder hasta çalışmaya dahil edildi. Hastalardan kan ve foliküler sıvı örnekleri oosit toplama (OPU) günü alındı. İMA düzeyleri Bar-Or ve ark. tarafından tanımlanan, kobaltın albümin ile bağlanmasını azaltan hızlı ve kalorimetrik bir teknikle manuel olarak ölçüldü. İstatistiksel yöntem olarak, t-testi veya Mann-Whitney U testi kullanıldı. Kategorik verilerin gruplar arasındaki frekans dağılımını belirlemek için ki-kare testi kullanıldı. Etki büyüklükleri, parametrik ve parametrik olmayan karşılaştırmalar için Cohen's d ve kategorik değişkenler için Cramér's V kullanıldı. Biyokimyasal parametrelerin korelasyonları için parametrik değerlerde Pearson korelasyon katsayısı ve parametrik olmayan verilerde Spearman korelasyon katsayısı kullanılmıştır. Bulgular: Her iki grup arasında demografik veriler, siklus parametreleri, klinik gebelik oranları arasında anlamlı fark yoktu (p>0.05). Kısa GnRH antagonist protokolde serum İMA değeri anlamlı olarak yüksek bulundu (p=0.043). Her iki tedavi grubunda da foliküler sıvı İMA düzeyleri ile gebelik oranları arasında korelasyon saptandı (r=0.042, r=0.056). Sonuç: Kısa GnRH antagonist protokolde serum İMA düzeyleri uzun GnRH analog protokolden yüksek olsa da en son nokta olan klinik gebelikler arasında her iki tedavi protokolünde anlamlı farklılık bulunmamıştır. Normoresponder hastaların IVF sikluslarında her iki protokolde tercih edilebilir. Oksidatif stres belirteci olan İMA düzeyleri erken dönemde IVF başarısının belirlenmesinde kullanılabilir.

Serum and follicular fluid ischemia modified albumin levels in agonist/antagonist controlled ovarian hyperstimulation regimens and their relationship with pregnancy rates

Abstract

Aim: We aimed to compare clinical pregnancy rates and cycle parameters with ischaemia-modified albumin levels (IMA), a marker of oxidative stress, in normoresponder patients receiving long GnRH analogue protocol or short GnRH antagonist protocol in in vitro fertilisation (IVF) cycles. Method: 60 normoresponder patients, 30 receiving the long GnRH analog protocol and 30 patients receiving the short GnRH antagonist protocol, were included. Blood and follicular fluid samples were taken from the patients on the oocyte pick up (OPU) day. IMA levels was measured manually using a rapid, calorimetric technique described by et Bar-Or et al., which reduces the binding of cobalt to albumin. As statistical method, t-test or Mann-Whitney U test was used. Chi-square test was used to determine the frequency distribution of categorical data between groups. Findings were obtained using Cohen's d for effect sizes, parametric and non-parametric comparisons, and Cramér's V for categorical variables. For correlations of biochemical parameters, Pearson correlation coefficient was used for parametric values and Spearman correlation coefficient for non-parametric data. Results: There was no significant difference in demographic data, cycle parameters, and clinical pregnancy rates between the two groups (p>0.05). Serum IMA value was found to be higher in the short protocol (p = 0.043). A correlation was detected between follicular fluid IMA and pregnancy rates in both treatment groups (r = 0.042, r = 0.056). Conclusion: Both protocols can be preferred in IVF cycles of normoresponder patients. IMA, a marker of oxidative stress, can be used to determine IVF success in the early period.
