PERİPARTUM DÖNEMDE SPLENİK ARTER ANEVRİZMA RÜPTÜRÜ: OLGU SUNUMU

Abstract

Splenik arter anevrizmaları (SAA), visseral arter anevrizmaları arasında en sık görülen anevrizma tipidir. SAA’nın rüptüre olması durumunda mortalitesi çok yüksektir. Kadınlarda daha sık görülen SAA, gebelerde fizyolojik olaylara bağlı olarak rüptür olma riski daha çok artar. Gebelerde SAA rüptürü, ani senkop ve şok tablosu ile klinikte karşımıza çıkar. Bu yazıda gebelerde çok nadir karşılaşılan SAA rüptür olgusunu literatür eşliğinde sunmayı amaçladık.Splenik arter anevrizmaları (SAA), visseral arter anevrizmaları arasında en sık görülen ve rüptüre olduğunda mortalite oranı yüksek olan bir patolojidir (1). En önemli komplikasyonu, anevrizma rüptürüdür (2, 3). Kadınlarda erkeklere oranla 4 kat daha fazla görülür (4). Etyolojik faktörler arasında travma, ateroskleroz, medial fibrodisplazi ve gebelikteki hemodinamik ve hormonal durumlar yer alır (5, 6). Hormonal durumlar gebelikte damar duvarlarını inceltir. Bundan dolayı rüptür oranı gebede daha fazla artar (7). Gebelikte spontan SAA rüptürü çok nadir görülür (8). Gebelikte SAA rüptürü daha çok karşımıza ani bayılma, hipovolemik şok ve ani ölümle çıkar. Bu klinik parametrelerinden dolayı, SAA rüptürünün tanısı uterus rüptürü ve dekolman plasentadan dolayı gecikebilir.
