Evaluation Of Urinary Findings In Patients Using Systemic Retinoid

Abstract

Introduction: It is well known that side effects of
systemic retinoids especially mucosal dryness of
mouth, nose and conjunctiva. The nasal bleeding
related nasal dryness is a common side effect of systemic retinoid. However mucosal dryness of

urethra related hemathuria reported with only one
case. In this study, we aimed to evaluate the urin
analyse findings of the patients who use systemic
retinoid.
Material and Method:
The last year’s data of the patients who were using
oral isotretinoin for moderate to severe acne and
oral acitretin for several dermatological disease
were screened retrospectively. Fifty-five patients
who were examined for urin-analyse during retinoic
acid treatment included to the study. Ages of
patients, types of retinoic acid, dosage of retinoic
acid, month of the retinoid treatment when urine
analyse examined; mucosal side effects, nail
ingrown were questioned. The laboratory tests and
urin analyse findings, were recorded. Forty age and
sex matched asymptomatic patients who were
examined for urinalysis were randomly selected for
control randomly from hospital information system.
Results: The 19 men and 34 women who were
using retinoic acids were included to the study.
Mean age of patients was 23.2±10.4years (10-64).
The19 men and 21 women were taken as control
group. Mean age of controls was 24.6±5.7years
(15-40)(p>0.05). The 49 patients were using
isotretinoin and 6 patients were using acitretin. The
results of urinalysis of patients were compared with
the control group and the difference was not
determined for the urine pH, urine density values
and presence of leukocytes and erythrocytes in
urine (p!0.05). The presence of red blood cells in
the urine associated with urinary tract infection in 4
patients. The presence of epithelial cells in the urine
was higher in retinoic acid group compared to the
control (p <0.05).
Conclusion: Higher ratios of epithelial cells
presence in the urine of retinoic acid using group
may be associated with dryness of urogenital
mucosa. Isolated hematuria without underlying
urological pathology was not detected in our patient
group. However, prospective studies of larger series
are needed in this regard.

SİSTEMİK RETİNOİD KULLANAN HASTALARDA İDRAR BULGULARININ DEĞERLENDİRİLMESİ

Abstract

Giriş: Sistemik retinoid kullanımının yan etkileri
arasında oral, nazal ve konjonktival mukoza
kuruluğu iyi bilinmektedir. Nazal mukoza
kuruluğuna bağlı burun kanaması sık görülen bir
yan etki olmakla birlikte üretral mukoza kuruluğuna
bağlı hematüri sadece bir olguda bildirilmiştir.
Çalışmamızda sistemik retinoid kullanan hastaların
idrar bulgularının değerlendirilmesi amaçlanmıştır.
Gereç ve yöntem: Çalışmamıza son bir yıl içinde
kliniğimizde orta ve şiddetli akne tanısı ile oral
isotretinoin kullanan ve çeşitli nedenlerle asitretin
tedavisi verilen hastaların verileri retrospektif
olarak tarandı ve tedavi sırasında tam idrar tetkiki
yapılan 55 hasta çalışmaya dahil edildi. Hastaların
yaşları, kullandığı retinoik asit tipi, retinoik asit
dozları, tam idrar tetkikinin yapıldığı tedavi ayı,
diğer mukozal yan etkiler, laboratuvar tetkikleri ve
tam idrar tetkiki bulguları kaydedildi.
Bulgular: Sistemik retinoid kullanan 19 erkek 34
kadın hasta çalışmaya dahil edildi. Hasta grubunun
yaş ortalaması 23.2±10.4 yıl (10-64) idi. Kontrol
grubuna 19 erkek 21 kadın alındı. Kontrol
grubunun yaş ortalaması ise 24.6±5.7 yıl (15-40)
(p>0.05) idi. Toplam 49 hasta isotretinoin, 6 hasta
ise asitretin kullanıyordu. Hastaların idrar analizi
sonuçları kontrol grubu ile karşılaştırıldığında idrar
PH, idrar dansite değerleri, idrarda lökosit ve
eritrosit varlığı açısından fark belirlenmedi
(p>0.05). İdrarda epitel varlığı retinoik asit kullanan
grupta kontrol grubuna oranla yüksek
bulundu(p<0.05).

Sonuç: İdrarda epitel varlığının sistemik retinoid
kullanan grupta yüksek oranda saptanması
ürogenital mukoza kuruluğu ile ilişkili olabilir.
Bizim hasta grubumuzda altta yatan ürolojik
patoloji olmaksızın izole hematüri saptanmadı.
Ancak bu konuda daha daha çok hasta ile yapılacak
prospektif çalışmalara ihtiyaç olduğunu
düşünmekteyiz.
