BÖLGEMİZDEKİ ADÖLESAN GEBELİKLERİN MATERNAL VE PERİNATAL SONUÇLAR ÜZERİNE ETKİLERİNİN DEĞERLENDİRİLMESİ

Abstract

Amaç: Adölesan terimi ile çocukluktan
erişkinliğe geçiş tanımlanmaktadır ve tüm dünya ülkelerinde adölesan gebelikler
önemli sağlık sorunu oluşturmaktadır. Adölesan gebeler maternal ve fetal açıdan
yüksek riskli gebeliklerdir. Çalışmamızda adölesan ve adölesan olmayan
gebelerin obstetrik ve neonatal sonuçlarını karşılaştırmayı amaçladık.



Gereç ve Yöntem: : Çalışmamızda
20 hafta üzerindeki doğum yapan 19 yaş ve altı gebeler ile aynı gebelik haftasında
doğum yapmış 20-29 yaş arası gebelerin, gebelik ve doğum kayıtlarını
retrospektif olarak inceledik. Grupların obstetrik sonuçlar (doğum şekli, doğum
kilosu) ve obstetrik komplikasyonlar (preterm doğum, fetal anomali, eklampsi)
açısından karşılaştırıldı.



Bulgular: Adölesan 256 gebe ile adölesan olmayan
772 gebenin dahil edildiği çalışmamızda adölesanlar ortalama 34.92±4.59 gebelik
haftasında, adölesan olmayanlar ise 35.71±4.25 gebelik haftasında doğum yaptığı
tespit edildi (p<0.05). Adölesan grupta eklampsi gelişiminin daha fazla
olduğu görüldü (p<0.05). Adolesan olanlarda çoğunlukla doğum şekli vajinal,
adolesan olmayanlarda ise sezaryen idi. (p<0.05). Bebek ağırlığının anne
yaşındaki küçülmeyle birlikte azaldığı tespit edildi (p<0.05 ). Fetal anomali
gelişimi açısından fark olmadığı gözlendi (p<0.05).



Sonuç: Adölesan gebelikler maternal ve
perinatal morbidite ve mortalite risk artışına neden olabilmektedir. Anne ve
çocuk sağlığı ile ilgilenen kliniklerin bu gebelerin sıkı antenatal takibi ve
riskleri konusunda uyanık olması gerekmektedir.
