Traumatic Tooth Injuries Evaluated as Forensic Cases

Abstract

Introduction and Aim: In the study, it was aimed to emphasize that the

lesion characteristics of patients with tooth fractures who applied to

hospital due to trauma should be examined and the traumatic tooth

fractures should be evaluated within the scope of judicial case.

Material and Method: The cases with traumatic tooth injuries, the

judicial reports of which were prepared, were retrospectively evaluated

by Eskişehir Forensic Medicine Branch Office between January 1, 2014

and December 31, 2014. The cases were classified according to age,

sex, event type and injury type.

Findings: It was determined that the average age of the cases was

17±12.3, 267 of them (85.6%) were male, the teeth of 157 cases

(50.3%) were broken due to battery, and the upper incisor teeth were

most frequently broken teeth. A report was drawn up on the fact that

the injury "could not be removed with a simple medical intervention" in

all cases in the existing application. However, in the application before

June 2013, it was determined that the lesion could be removed by a

simple medical intervention in 179 (57.4%) cases with crown fracture

when lesions outside tooth fractures were ignored, and it could not be

removed by a simple medical intervention in 133 (42.6%) cases with

root fracture. The men wounded at a young age, it was found that

women injured in older age. The most frequent reasons of tooth

fracture which has been occurred in men and women are separated.

Men’s cases are the most frequent reason is fight and also women’s

cases the most frequent is traffic accident.

Conclusion: It is shown that the update made in the guide is quite

important for tooth injuries. The demographic data of cases were found

to be consistent with the literature outside of the origin. However, it is

thought that the teeth traumas alleged to have occurred accidentally

are not treated as judicial case. The in-service training regarding

forensic medicine approach is thought to be useful especially for

dentists offering emergency services.

ADLİ OLGU OLARAK DEĞERLENDİRİLEN TRAVMATİK DİŞ YARALANMALARI

Abstract

Giriş ve Amaç: Çalışmada travma nedeni ile başvuran diş kırıklı

olgulara ait lezyon özelliklerinin irdelenmesi, travmatik diş kırıklarının

adli vaka kapsamında değerlendirilmesi gerektiğinin vurgulanması

amaçlanmıştır.

Gereç ve Yöntem: Eskişehir Adli Tıp Şube Müdürlüğü’nce 1 Ocak 2014

ile 31 Aralık 2014 tarihleri arasında adli raporları düzenlenen travmatik

diş yaralanmalı olgular retrospektif olarak değerlendirilmiştir. Olgular

yaş, cinsiyet, olay türü ve yaralanma niteliğine göre sınıflandırılmıştır.

Bulgular: Olguların yaş ortalamasının 17±12,3 ve 267’sinin (% 85,6)

erkek olduğu, 157 olgunun (% 50,3) darp nedeniyle dişinin kırıldığı, en

sık üst santral dişlerin kırıldığı belirlenmiştir. Mevcut uygulamada

olguların tümünde yaralanmanın “basit tıbbi müdahale ile

giderilemeyeceği” yönünde rapor düzenlenmiştir. Ancak 2013 Haziran

öncesindeki uygulamada diş kırıkları dışındaki lezyonlar göz ardı

edildiğinde kron kırığı olan 179 (% 57,4) olguda lezyonun basit tıbbi

müdahale ile giderilebileceği, kök kırığı olan 133 (% 42,6) olguda ise

basit tıbbi müdahale ile giderilemeyeceği saptanmıştır. Erkek olguların

daha küçük yaşlarda yaralandığı, kadın olguların daha büyük yaşlarda

yaralandığı tespit edilmiştir. Erkek olgularda daha sık etkili eylem, kadın

olgularda ise daha sık trafik kazası nedeniyle diş kırığı olduğu

belirlenmiştir

Sonuç: Kılavuzda yapılan güncellemenin, diş yaralanmaları açısından

oldukça önemli olduğu görülmektedir. Orijin dışında olguların

demografik verileri literatürle uyumlu bulunmuştur. Ancak kaza sonucu

oluştuğu iddia edilen diş travmalarının adli vaka yapılmadığı

düşünülmektedir. Özellikle acil hizmeti veren diş hekimlerine adli tıbbi

yaklaşım ile ilgili hizmet içi eğitimin yararlı olacağı düşünülmektedir.
