Evaluation of Mandibular Bone Structure of Patients Who Using Systemic Glucocorticoid with Fractal Analysis

Glucocorticoids are drugs used in the treatment of many inflammatory, allergic and autoimmune diseases. Glucocorticoid-induced osteoporosis is the most common cause of secondary osteoporosis, at the same time is the most common cause of iatrogenic osteoporosis. The dominant effect of glucocorticoids on the bone structure is the deterioration of bone formation. Reduced bone mineral density is one of the rapid effects of glucocorticoids. Fractal analysis is a method to describe complexity of trabecular bone structure. Fractal analysis is expressed numerically with fractal dimension (FD). The aim of this study, evaluation of mandibular bone structure of patients who using systemic glucocorticoid with fractal analysis. Panoramic radiographs of the patients who used systemic glucocorticoids were retrospectively scanned. Trabecular structure of mandible of patients who uses glucocorticoid was compared to control group patients using fractal analysis method. In our study, there were 69 who used glucocorticoids and 69 control patients. 4 different areas were selected manually for measurements on panoramic radiographs. The FD was calculated with the ImageJ program. Results: According to the results of FD measurements there was no statistically significant difference between the two groups in the ROI, ROI2, and ROI3 localizations. (p=0.227, 0.268, 0.271) There was a statistically significant difference between the measurements in ROI4. (p=0.046) The mean of FB measurements in ROI4 was significantly lower than the control group. High FD value presents trabecular bone density and complexity of trabecular structure. Fractal analysis is an efficient method to evaluate the effect of drugs that effect bone turn over on jaws.

Sistemik Glukokortikoid Kullanan Hastaların Mandibular Kemik Yapısının Fraktal Analiz ile Değerlendirilmesi

Öz

Glukokortikoidler, güçlü antiinflamatuar, antiallerjik, immünsüpresif etkileri nedeniyle birçok inflamatuar, allerjik ve otoimmün hastalığın tedavisinde kullanılan ilaçlardır. Glukokortikoid bağımlı osteoporoz, sekonder osteoporozun ve 50 yaş altında görülen osteoporozun en sık nedeni olmakla birlikte osteoporozun en sık iyatrojenik nedenidir. Glukokortikoidlerin kemik üzerindeki baskın etkisi, kemik formasyonunun bozulmasıdır. Kemik mineral yoğunluğunda azalma glukokortikoidlerin hızlı etkilerindendir. Fraktal analiz (FA), trabeküler kemik gibi karmaşık yapıların incelenmesinde kullanılan matematiksel bir ölçüm tekniğidir ve sayısal olarak fraktal boyut (FB) değeriyle ifade edilir. Bu çalışmanın amacı sistemik glukokortikoid kullanan hastaların mandibulalarının FA metoduyla değerlendirilmesidir. Anamnez kayıtları baz alınarak sistemik glukokortikoid kullandığı tespit edilen hastaların panoramik radyografları restrospektif olarak tarandı. FA metodu kullanılarak glukokortikoid kullanan hastaların mandibular kemiklerinin trabeküler yapısı kontrol hastalarıyla karşılaştırıldı. Çalışmamızda 69 glukokortikoid kullanan hasta ve 69 sistemik olarak sağlıklı kontrol hastası mevcuttu. Panoramik radyograflar üzerinde ölçümler için 4 farklı alan ramus (İA1), angulus (İA2), mental foramen önü (İA3), mandibular kortikal alan (İA4) manuel olarak seçildi. FB, ImageJ programı ile hesaplandı. Bulgular: Hasta ve kontrol gruplarında dört farklı lokalizasyona ait FB ölçümleri sonucuna göre, İA1, İA2, İA3 lokalizasyonlarında iki grup arasında istatistiksel olarak anlamlı bir fark yoktu. (p=0.227, 0.268, 0.271) İA4’te yapılan ölçümler arasında ise istatistiksel olarak anlamlı bir fark vardı. (p=0.046) Hasta grubunda İA4’teki FB ölçümlerinin ortalaması, kontrol grubuna göre daha düşüktü. FB değerinin yüksek olması kemik trabekülasyon yoğunluğu ile doğru orantılı bir veridir. Bu çalışmada glukokortikoid kullanan hastalarda İA4’te FB’de anlamlı azalma gözlenmiştir. Mandibular kemik dokusunda değişikliklere yol açabilen ilaç kullanan hastalarda kemikte trabekülasyon kayıplarını değerlendirmede FA kullanılabilecek bir yöntemdir.
