Abstract

Genelde kanserin korkunç bir
hastalık olduğu düşünülür, yalnızca adı bile ürkütücüdür. Çoğu insan kanserden
korkar, çünkü bunun tedavi edilemez ve ölümcül bir hastalık olduğuna
inanmaktadır. Kanser hastalığının doğası gereği hasta ve çevresinde oluşturduğu
ciddi endişe ve stres, bu hastalığın tedavisi ile ilgilenen bütün sağlık
çalışanlarının üzerinde önemli bir baskı oluşturmakta ve erken dönemde
yıpranma, iş motivasyonu ve verimlilik kaybına neden olabilmektedir. Merhamet
bir kimsenin veya bir başka canlının karşılaştığı kötü durumdan dolayı duyulan
üzüntü, acıma olarak tanımlanmıştır. Sağlık çalışanlarında merhamet yorgunluğu,
ciddi duygusal ya da fiziksel acı içerisindeki hastalara gösterilen merhametin
zamanla aşamalı olarak azalması ifade edilmiştir. Literatüre bakıldığında,
merhamet yorgunluğu ile ilgili hemşireler ve diğer hasta bakımı sağlayan sağlık
çalışanları ile ilgili birçok çalışma olduğu ancak cerrahi yapan hekimler
üzerinde yapılan çalışmaların sınırlı olduğu görülmektedir. Özellikle kanser
cerrahisi yapan hekimlerde, hastalığın tanı, tedavi ve takibinin uzun ve yorucu
olması bu hekim grubunda merhamet yorgunluğu oluşmasına neden olabilir. Bu
çalışmada baş boyun cerrahisi ile ilgilenen 100 hekime Pommier tarafından
geliştirilen, Akdeniz ve Deniz tarafından Türkçe’ye çevrilen 24 soruluk “Merhamet
Yorgunluğu Ölçeği” uygulanmış ve ölçek toplam puan ortalaması 4.24±0.97 olarak
bulunmuştur. Bu sonuç, ölçekten alınan puanın en fazla 5 olması nedeniyle, baş
boyun cerrahlarında merhamet düzeyinin yüksek olduğu yönünde yorumlanmıştır. Bu
çalışma sadece baş boyun kanseri ile uğraşan cerrah grubunu kapsaması nedeniyle
literatürde ilk olduğu gibi, sadece kanser ameliyatları yapan hekim grubunda merhamet
yorgunluğunu araştırması nedeniyle de literatürde bir ilktir. Çalışmadan elde
edilen sonuçlar tıpta en korkulan hastalıklardan biri olan kanser ile uğraşan
hekimlerin düşünülenin aksine merhamet yorgunu olmadıklarını ve mesleklerini
başarı ve özveri ile sürdürmeye çalıştıklarını ortaya koymaktadır.

Compassion Fatigue in Health Care Personnel

Abstract

Even the very mention of cancer is enough to frighten most people, as many
believe that it is an incurable disease that usually ends in death. As a
natural result of this situation, a cancer diagnosis causes anxiety to the
patient, the family and any health care personnel who are involved in the
patient's diagnosis and treatment. This pressure leads to exhaustion in the
first stages, and loss of motivation and decreased productivity. Compassion may
be defined as sadness and pity felt for a person who is in a poor condition.
Compassion fatigue is a state experienced by healthcare providers who have been
helping people for extended periods. Although there are many studies related to
compassion fatigue in the literature, there are limited numbers of studies in
existence concerning cancer surgeons. As the complete sequence of diagnosing
cancer, informing the patient and planning treatment involves an extreme level
of tension, compassion fatigue might be anticipated in this group of doctors.

We applied the Compassion Scale,
which was initially designed by Pommier and later adapted for Turkish by
Akdeniz and Deniz, to one hundred doctors who perform head and neck cancer
surgery on a regular basis. The final mean score obtained was higher than our
expectations, at 4.24±0.97 out of 5. This result can be interpreted as showing
surgeons have high levels of compassion, without any corresponding fatigue.
According to the results of our literature search, this is the first study
related to compassion in head and neck surgeons and it is very gratifying to
observe that they are still working with devotion and precision.
