Maksilla kanserleri; Üç Yanlış Teşhis Vakası ve Literatür Taraması

Abstract

Maksilla kanserleri nadirdir ve anatomik sınıflandırmada oral kanserlerin %0.5-5'ini oluşturur. Birçok çalışma ağız boşluğunu araştırmıştır ancak maksiller karsinomu araştıran az sayıda çalışma vardır. Ayırıcı tanılar iyi bilinmezse teşhis koymak gecikebilir ve yanlış teşhis sonucu zaman kaybına hastalığın seyri değişebilir ve tedavi aksar. Bu retrospektif çalışmada maksilla kanserlerinin teşhis ve tedavisinde öneriler getirmek, aynı zamanda diş hekimliği açısından önemine dikkat çekmek amaçlanmıştır. Uzun süre dental problemleri olduğu zannedilen yanlış teşhis koyulmuş ve buna bağlı olarak hatalı tedavi edilmiş 3 kadın hastanın klinik, radyolojik ve histopatolojik tanıları ile predispozan faktörleri belirlendi. Kanser tedavisinden önce dental değerlendirmeleri yapıldı. Sağ kalım, yaşam kalitesi ve tedavi literatürdeki diğer olgularla karşılaştırıldı. Hastaların yaşları 36, 57 ve 59 idi ve tüm hastalara maksillektomi yapıldı. Histopatolojik olarak adenokistik karsinom ve SCC saptandı. İki hasta ameliyat öncesi radyoterapi ve kemoterapi ile tedavi edilirken, bir hastaya ameliyat sonrası adjuvan tedavi uygulandı. Radyoterapi almayan hastaya obturator yapıldı. Hastalar literatürde olduğu gibi tedavi edilmiştir ve sağ kalımları yüksektir. Hastaların ikisinde sigara ve alkol gibi predispozan faktörlerin olmaması ve kanser teşhisi öncesinde dental sorunlara odaklanılması ayırıcı tanının önemini ortaya koymaktadır. Diş hekimlerinin maksilla kanserinin teşhis ve tedavisindeki rolü büyük olup bu konuda paylaşılacak her olgunun katkı sağlayacağı düşünülmektedir.

Cancers of the Maxilla; Three Misdiagnosis Cases and Review of the Literature

Abstract

Cancer of the maxilla is uncommon and constitutes 0.5-5% of oral cancers on anatomical classification. Many studies have researched the oral cavity, but there are few studies investigating maxillary carcinoma. If differential diagnoses are not well known, the diagnosis may be delayed and the misdiagnosis may result in loss of time and the course of the disease may interfere with treatment. In this retrospective study, it was aimed to make recommendations in the diagnosis and treatment of maxillary cancers and also to take attention to its importance in terms of dentistry. Clinical, radiological and histopathological diagnosis and predisposing factors of 3 female patients who had been misdiagnosed and thought to have dental problems for a long time were determined. Dental evaluations were performed before cancer treatment. Survival, quality of life and treatment were compared with other cases in the literature. The ages of the patients were 36, 57 and 59 and all patients underwent maxillectomy. Adenocystic carcinoma and squamous cell carcinoma were detected histopathologically. Two patients were treated preoperatively via radiotherapy and chemotherapy, whereas one patient received postoperative adjuvant therapy. The obturator was applied to the patient who did not receive radiotherapy. The patients were treated as in the literature and their survival was high. The lack of predisposing factors such as smoking and alcohol in two patients and focusing on dental problems before the diagnosis of cancer reveal the importance of differential diagnosis. The role of dentists in the diagnosis and treatment of maxilla cancer is great and it is thought that the contribution of each case will be shared.
