Rehabilitation with Fixed Hybrid Restoration of an Edentulous Patient with Mandibular Reconstruction: A Case Report

Abstract

In this case report, the restoration of a segmental mandibular defect which occurred after the resection of a recurrent squamous cell carcinoma lesion with implant-supported hybrid prosthesis after reconstructed with free fibular flap was described. A 48-year-old male patient with partial glossectomy and mandibular reconstruction was referred to clinic due to maxillary and mandibular partial-edentulism. Clinical and radiological examinations revealed teeth with decreased bone support because of chronic-periodontitis and maxillary impacted canine. After extraction of impacted canine and other teeth, a wax-up try-in was performed to determine final teeth position and vertical dimension. Due to significant vertical tissue loss, hybrid-prostheses were planned. Four implants were inserted to each jaw. After osseointegration, open-tray impressions were taken, metal-frameworks were fabricated, and their passive fit were checked clinically. Centric relation, vertical dimension, phonetic and aesthetic of tooth setting were verified intraorally. After occlusal adjustments, the final prostheses were delivered to patient and screw holes were covered. The patient was satisfied with stability and esthetics of his prostheses and did not have any problems regarding prostheses during 6 months of service. The standard treatment approach for oral cancers is the surgical resection of pathological tissues with a certain sum of peripheral healthy tissue and reconstruction of hard and soft tissue defects secondary to the excision using different grafts. However, due to inadequate tissue support after reconstruction, conventional prosthetic restorations cannot have sufficient retention and function. Therefore, implant-supported hybrid prostheses represent good alternative for edentulous jaws with significant hard and soft tissue loss.

Mandibular Rekonstrüksiyonlu Dişsiz Hastanın Sabit Hibrit Protezle Rehabilitasyonu: Olgu Sunumu

Abstract

Bu olgu sunumunda skuamöz hücreli karsinomun sol mandibular posterior bölgede nüksü sebebiyle ortaya çıkan lezyonun cerrahi rezeksiyonu nedeniyle oluşan mandibular segmental defektin, serbest fibula flebi ile rekonstrüksiyonunu takiben implant-destekli hibrit protezle rehabilitasyonu anlatılmıştır. Parsiyel glossektomi ve mandibular rekonstrüksiyon geçirmiş 48 yaşında erkek hasta maksiller ve mandibular parsiyel dişsizlik nedeniyle kliniğimize yönlendirildi. Klinik ve radyolojik muayenelerde kronik periodontitis sebebiyle kemik desteği azalmış dişler ve maksiller gömülü kaninin mevcut olduğu gözlendi. Öncelikle gömülü kanin ve mevcut dişlerin çekimi gerçekleştirildi. Alınan ilk ölçü yardımıyla elde edilen modele kaide plağı hazırlanarak mumlu prova ve diş dizimi yapılarak hastanın dikey boyutu ve diş pozisyonları tespit edildi ve vertikal doku kaybının fazla olduğu görülen hastaya hibrit-protez planlandı. Maksillaya ve mandibulaya 4’er implant yerleştirildi. Osseointegrasyonun ardından, açık kaşık tekniğiyle ölçü alınıp, modellerde metal altyapı hazırlandı ve ağızda pasif uyumu kontrol edildi. Dişli provada sentrik ilişki, dikey boyut, fonetik ve estetik kontrolü yapıldı. Bitirilen protez oklüzal uyumlamaları yapıldıktan sonra vida girişleri kompozitle kapatılarak hastaya teslim edildi. Hasta protezinin stabilitesi ve estetiğinden memnun kaldı ve 6 aylık kullanımı süresince herhangi bir problem yaşamadı. Oral kanserlerde standart tedavi yaklaşımı patolojik dokuların çevredeki bir miktar sağlıklı doku ile uzaklaştırılması ve lezyonun eksizyonel cerrahisine sekonder oluşan sert ve yumuşak doku defektlerinin çeşitli greftlerle rekonstrüksiyonudur. Ancak, rekonstrüksiyon sonrası yetersiz doku desteği nedeniyle, klasik yöntemlerle yapılan protetik restorasyonlar yeterli tutuculuk ve fonksiyon sağlayamamaktadır. Bu sebeple sert ve yumuşak doku kaybının fazla olduğu dişsiz kretlerde implant-destekli hibrit protezler iyi bir alternatif oluşturmaktadır.
