Devolusyon ve Sağlık Sonuçları

Abstract

Seksenli yıllardan sonra neoliberal ekonomik politikalar, küreselleşme ve siyasal gelişmelerle birlikte devletin kamu hizmetlerindeki rolünün sınırlandırılması amaçlanmış ve dünyanın çoğu ülkesinde devletin merkezi işlevi kapsamındaki bazı yetkiler yerel birimlere aktarılmıştır. Bu yetki aktarımı desantralizasyon kavramıyla açıklanmıştır. Bir desantralizasyon türü olan devolusyon ise merkezi yetkilerin bölgeler, belediyeler gibi alt düzey siyasi makamlara aktarılması, aktarılan bu yetkilerle merkezi hükümete bağlı yerel ve özerkliği olan yapılar oluşturulmasıdır. İncelenen çalışmalarda devolusyonun yerel yönetim bölgelerinde; kamusal sağlık hizmetinden memnuniyeti, sağlık göstergelerinde (bebek ölüm hızı, doğum öncesi bakıma erişim, doğumda beklenen yaşam süresi) iyileşmeyi, gelir düzeyini ve pandemi ve afeteler gibi olası halk sağlığı krizlerine müdahale kapasitesini artırdığı belirlenmiştir. Ancak bu artışlar sağlanırken yerel yönetim bölgeleri arasındaki gelir ve sağlık eşitsizliklerinden etkilenildiği ya da bu tip eşitsizliklere neden olduğu da gözlenmiştir. Çalışmamızda devolusyonun uygulandığı farklı ülkelerin sağlık sonuçları incelenmiş ve devolusyon uygulanan bölgelerde sağlık sonuçlarının iyileştirilebilmesi için çözüm önerileri sunulmuştur.

Devolution and Health Outcomes

Abstract

After the 1980s, with the neoliberal economic policies, globalization and political developments, it was aimed to limit the role of the state in public services, and in most countries of the world, some powers within the scope of the central function of the state were transferred to local units. This transfer of authority is explained with the concept of decentralization. Devolution, which is a type of decentralization, is the transfer of central powers to low-level political authorities such as regions and municipalities, and the creation of local and autonomous structures affiliated to the central government with these transferred powers. In the studies examined, in the local government regions of devolution; satisfaction with public health services, improvement in health indicators (infant mortality rate, access to prenatal care, life expectancy at birth), income level and capacity to respond to possible public health crises (pandemics and disasters). However, while these increases were achieved, it was observed that the income and health inequalities between the local government regions were affected or caused such inequalities. In our study, the health outcomes of different countries where devolution was applied were examined and solutions were presented to improve health outcomes in the regions where devolution was applied.
