Klinikten Ameliyathaneye Güvenli Cerrahi Kontrol Listesinin Doldurulma Oranları: Retrospektif Çalışma

Abstract

Giriş: Güvenli Cerrahi Kontrol Listesi GCKL-TR) ameliyat sürecinin koordinasyonunu sağlamada hemşire için yol göstericidir. Amaç: Cerrahi kliniğinden ameliyathaneye teslim edilen hastalarda GCKL-TR doldurulma durumunun incelenmesidir. Gereç ve Yöntem: Araştırma tanımlayıcı, retrospektif tipte bir araştırma olup 1-31 Aralık 2021 tarihleri arasında İzmir’de özel bir hastanenin ameliyathane kayıtları incelenerek gerçekleştirildi. Verilerin toplanmasında GCKL-TR, ameliyat onamı, Eksik Hazırlıkla Ameliyathaneye Transfer Edilen Hasta Oranı İndikatör Takip Formu retrospektif olarak incelendi. Bulgular: Araştırmada 11 farklı cerrahi klinikten toplam 2471 hastanın dosya taraması yapıldı. Hastaların tamamında (n=2471, %100) kimlik bilgilerinin, ameliyatının, ameliyat bölgesinin doğrulandığı görüldü. Hastaların tamamının (n=2471, %100) ameliyathaneye onamı alınarak ve onam belgesi dosyasına koyularak gönderildiği, tüm hastaların onamında “barkod-isim soyisim- imza-tarih saat” kısımlarının doldurulduğu görüldü. Hastaların tamamı ameliyathaneye açlık protokolüne uygun olarak teslim edildi. Hastaların tamamının (n=2471, %100) ameliyathaneye gönderilmeden önce kan ve kan ürünü gereksinimi, laboratuvar ve radyoloji tetkikleri teyit edildi. Sadece beş dosyada (%0,02) GCKL-TR’nde hemşire ad soyad imza kısmında eksik görüldü. Sonuç ve Öneriler: Çalışmada GCKL-TR doldurulma oranları yüksek bulundu. Hemşirelerin GCKL-TR konusundaki bilgi düzeyleri ve hasta güvenliğine katkısı konusundaki inancı bu konudaki önemli faktörlerden biri olarak düşünülmektedir.

Clinical-to-Operating Room Safe Surgery Checklist Completion Rates: A Retrospective Study

Abstract

Introduction: Safe Surgery Checklist (GCKL-TR) is a guide for the nurse in ensuring the coordination of the surgical process. Aim: To examine the completion of the GCKL-TR in patients who were delivered to the operating room from the surgery clinic. Material and Methods: This is a descriptive, retrospective study and was conducted between 1-31 December 2021 in the operating room of a private hospital in Izmir. In data collection, GCKL-TR Form, operative consent, Rate of Patients Transferred to the Operating Room with Incomplete Preparation Indicator Follow-up Form were reviewed retrospectively. Results: A total of 2471 patients from 11 different surgical clinics were scanned. It was seen that all the patients' identity information, surgery, and operation area were confirmed. It was observed that all the patients (n=2471, %100) were sent to the operating room after taking their consent and included in the consent document file, and “barcode-name-surname-signature-date-time” sections were filled in in the consent of all patients. All the patients (n=2471, %100) were delivered to the operating room in accordance with the fasting protocol. Before all patients were sent to the operating room, blood and blood product requirements, laboratory and radiological examinations were confirmed. In only five files (0.02%), the nurse's name and surname signature was missing in the GCKL-TR. Conlusion and Sugestions: GCKL-TR filling rates were found to be high in the study. Nurses' level of knowledge on GCKL-TR and their belief in its contribution to patient safety are considered to be one of the important factors in this regard.
