Acil Serviste Çalışan Hemşirelerin Pulse Oksimetre Bilgisine Eğitimin Etkisi

Abstract

Giriş: Pulse oksimetre acil servislerde kullanılan, kalibrasyon gerektirmeyen, invazifişlem sayısını azaltan standart bir araçtır. Amaç: Acil serviste çalışan hemşirelerin, hasta izleminde kullandıkları pulse oksimetre ile ilgili bilgi düzeyine eğitimin etkisini belirlemektir. Gereç ve Yöntemler: Tek grupta ön test-son test düzenli yarı deneysel tasarım kullanılan araştırmanın örneklemini, İstanbul’da bir eğitim hastanesinin acil servisinde çalışan toplam 30 hemşire oluşturdu. Araştırmada kullanılan anket formunda hemşirelerin tanıtıcı özellikleri ve pulse oksimetre kullanımına ilişkin bilgi durumlarını belirlemeyi amaçlayan sorular yer aldı. Hemşirelerin bilgi durumunu ölçmeye yönelik anket, eğitim öncesi ve sonrası uygulandı. Veri analizinde; tanımlayıcı istatistikler, bağımlı gruplarda t-testi, bağımsız gruplarda t-testi ve tek yönlü varyans analizi kullanıldı. Bulgular: Eğitim öncesinde hemşirelerin, pulse oksimetrenin genel özellikleri ile ilgili bilgilerden oksijen saturasyonunun normal sınırlarını (%96.7) ve komplikasyonlarını (%73.3); pulse oksimetrenin hatalı okunmasına neden olan hasta kaynaklı hatalardan kardiyak arresti (%63.3); uygulama kaynaklı hatalardan hasta transferi sırasında parmak probunun güvenilir olmaması (%53.3) ve ortamda florasan/parlak ışık olması (%63.3) gibi bilgileri daha fazla oranda yanlış bildikleri belirlendi. Eğitim sonrasında hemşirelerin pulse oksimetrenin genel özellikleri, hasta ve uygulama kaynaklı hatalar ve toplam puanının, eğitim öncesine göre anlamlı olarak arttığı saptandı (p<0.05). Hemşirelerin tanıtıcı özelliklerinden sadece acil serviste çalışma yılının pulse oksimetre bilgi puanını etkilediği, acil serviste 6-10 yıl arasında çalışan hemşirelerin bilgi puan ortalamasının 3-11 ay ve 1-5 yıl arasında çalışanlardan anlamlı olarak daha düşük olduğu saptandı (p<0.05). Sonuç ve Öneriler: Araştırma, eğitimin acil serviste çalışan hemşirelerin pulse oksimetre bilgi puanını anlamlı olarak arttırdığını ve hatalı pulse oksimetre ölçümlerine neden olabilen yanlış uygulamaların önlenmesinde etkili olduğunu ortaya koydu.

The Effect of Training to Nurses Working at Emergency Department on Pulse Oximetry Knowledge

Abstract

Introduction: Pulse oximetry is a standard tool used in emergency services that does not require calibration and reduces the number of invasive procedures. Aim: This study aimed to determine the effect of training on emergency department nurses’ pulse oximetry
knowledge they apply in monitoring patients. Matreials and Methods: The study used a
group pre-test post-test quasi-experimental design and enrolled 30 emergency department
nurses employed in a training hospital in Istanbul. A questionnaire form was used to
determine the nurses’ descriptive characteristics and practical pulse oximetry knowledge
before and after training. Data were analysed using descriptive statistics, dependent and
independent samples t-tests, and one-way analysis of variance. Results: Pre-training results
showed that the nurses’ pulse oximetry knowledge needed improving most about normal
limits of oxygen saturation (96.7%) and complications (73.3%) in information about general
features of pulse oximetry, cardiac arrest (63.3%) in patient-induced errors causing
inaccurate pulse oximetry readings, and finger probe failure during patient transfer (53.3%)
and fluorescent/bright light (63.3%) illumination in application-induced errors. According to
post-training results, the nurses’ total scores improved significantly in general features of
pulse oximetry, patient- and application-induced errors (p<0.05). Among the nurses’
descriptive characteristics, only emergency department employment duration affected pulse
oximetry knowledge score, nurses with 6-10-year emergency departmentexperience
received significantly lower knowledge scores than those with 3-11-month or 1-5-year
employment (p<0.05).Conclusion and Suggestions: The study showed that training
significantly increased the emergency department nurses’ pulse oximetry knowledge scores
and was effective in preventing malpractices that may cause inaccurate pulse oximetry
readings.
