Gastrointestinal Sistem Malignitelerinde Preoperatif Radyolojik Görüntülemenin Rolü

SüleymanDemirel Üniversitesi TIP FAKÜLTESİ DERGİSİ: 2000 Haziran; 7(2) Gastrointestinal Sistem Malignitelerinde Preoperatif Radyolojik Görüntülemenin Rolü Koray Okur Recep Çetin Meltem Çetin Mahmut Bülbül Özet Günümüzde büyük morbidite ve mortaliteye sahip ganlrointestinal sistem malignitelerinin tedavi ve prognozunu belirlemede preoperatif dönemde yapılan doğru bir evreleme kuşkusuz çok önemli bir rol oynamaktadır., Bu çalışmadaki amacımız hastanemizin kuruluş aşamasında gastrointestinal sistem malignitesi nedeniyle ameliyat edilmiş hastalarımızın retrospektif olarak preoperatif tanı yöntemlerinin (transabdominal ultrasonografı, bilgisayarlı tomografi, magnetik rezonans görüntüleme (MRG), endolüminal ultrasonografı ve intraoperatif ultrasonografı) değerlerini ortaya koymaktı. Preoperatif evrelemede kullanılan yöntemlerin sonuçları [spesifisite, semitivite, pozitif prediküf değer(PPD), negatif prediktif değer (NPD) ve doğruluk oranları (DO)] literatür bilgileri ışığında 45 kadın, 47 erkek olmak üzere toplam 92 hastada değerlendirildi. Gastrik malignitelerde özofagus-mide-duedonum grafısi ve endoskopinin tanısal değeri bilinmektedir. Bilgisayarlı tomografi (BT) ve ultrasonografı (US) ile evrelemede bu yöntemlerin etkinliği, tümör (T) ve metastaz (M) açısından literatürle uyumlu, nodal tutulum (N) açısından düşük bulundu. Kolorektal malignitelerde de çift kontrast kolon grafısi ve rektosigmoidoskopi tanısal değer taşımaktadır. Bu grup malignitelerde BT sonuçlarımız literatürle uyumludur. US sonuçlarımızda N düşük bulunurken, M saptaması BT'ye göre daha üstündür. Preoperatif inceleme yöntemlerinin tanısal doğruluğu, gelişmiş cihazların kullanılması, kullanıcıların deneyimli olması ve uygun çalışma protokollerinin seçilmesiyle arttırılabilir. Anahtar Kelimeler: gastrointestina! sistem, malignensi, görüntüleme Abstract The Role of Preoperative Radiologic Imaging in Gastrointestinal System Malignities Currently, gastrointestinal tract malignancies have become important cause of morbidity and mortality. Preoperatif precise staging ofthe.se malignancies do have ver)' important role to determine treatmenl prolocols and prognosis. Ejfective preoperative staging of these malignancies could be achived by using such procedures like transabdominal US, CT, MRI, endolüminal US, intraoperative US. in these study, \ve ainıed (o evaluate preoperative effeclivness ofdiagnostic procedures by relrospective study ofour operated patients with gastrointestinal malignancy. The results of the tools used in preoperative staging ( specifıcity, sensitivity, positive predictive value, negative predictive value, diagnostic accuracy ) were evaluated w ith the guidance of literatüre knowledge in 45 female, 47 ınale, total 92 patients. Esophagus-stomach-duedonum x-ray graphies and endoscopy were found to be valuable diagnostic tools in gastric malignancies. The results of staging with CT and US were found to be in accordance with literatüre knowledge in T and M but not in N. We also observed, the importance of double contrast colon graphy and rectosigmoidoscopy as diagnostic tools in colorectal malignancies. The results of CT seans were found to be in accordance with literatüre where as US evaluation yielded Iow N detection and high M detecüon in comparison to CT. Diagnostic accuracy of the preoperative imaging can be increased \vith sophisticaîed devices, experienced users, and selection oftheproper methods.
