Özet

Giriş: Postoperatif analjezi uygulamasında nonopioid analjezik uygulaması yakın zamanlarda yaygın şekilde kullanılmaktadır. Böylelikle opioid ilaç gereksinimi ve bunlara bağlı yan etkilerde azalabilmekte-dir. Tenoksikamda bu amaçla kullanılan bir ajandır. Bu çalışmada total abdominal histerektomi yapılan olgularda postoperatif analjezi uygulamasında tenoksikam etkinliği araştırıldı.

Metot: 100 ASA 1-11 olgu randomize olarak 50'şer olgu içeren iki gruba ayrıldı. Tüm olgularda stan¬dart genel anestezi uygulandı. Grup l olgulara (tenoksikam+meperidin) derlenme odasında 20 mg interavenöz tenoksikam uygulandı. Grup 2 olgulara (Meperidin) 6 saatte bir rutin olarak meperidin 50 mg uygulandı. Olgular 12 saat süresince nümerik skala ile değerlendirildiler(0:Ağrı yok, 5: dayanılmaz ağrı). Ek ilaç gereksinimleri 1., 6., ve 12.saatlerde kayıt edildi. Sonuçlar istatistiksel olarak kayıt edildi; p<0.05 anlamlı kabul edildi.

Sonuçlar: Yaş ve kilo yönüyle anlamlı farklılık olmadı. Birinci saatte Grup l'de 17 olguda ve Grup2'de 30 olguda analjezi gereksinimi oldu. Altıncı saatte Grup l'de 20 olguda ve Grup2'de 30 olguda analjezi gereksinimi oldu. On ikinci saatte Grup l'de 13 olguda ve Grup2'de 25 olguda analjezi gereksi¬nimi oldu.

Tartışma: Opioid ve nonopioid ilaç kombinasyonu postoperatif ağrı tedavisinde etkin olmaktadır. Yapılan bazı çalışmalarda tenoksikamın antienflamatuar etkisiyle, her hangi bir yan etki yaratmaksızın opioid gereksinimini azalttığı gösterilmiştir. Çalışmamızda da meperidin ve tenoksikam kombinasyonunun etkin olduğu ve meperidin gereksinimini azalttığı sonucunu elde ettik.


Abstract


Tenoxicam For Postoperative Analgesia After Total Abdominal Hysterectomy

Introduction: Recently nonopioid drug administration has been \videly used in postoperative pain management. The reasons for this are the reduce opioid dosage required and to eliminate the side effects. Tenoxicam is öne of the agents used for this purpose. in this study, effectiveness was evaluated when used for postoperative pain management atter total abdominal hysterectomy (TAH).

Methods: 100 ASA I-II patients were randomly separated into two groups of 50 patients. Ali patients received standard general anesthesia. Group l patients (tenoxicam+meperidine) received 20-mg intravenous (I.V). tenoxicam in the recovery room; îf the pain score was 2 ör över atter öne hour 50 mg meperidine was administered. Group 2 patients (Meperidine) received 50 mg meperidine intramuscular (İM) every 6 hours routinely in the ward. Patients were evaluated for 12 hours for postoperative pain using a numerical rating scala (0:no pain, 5: unbearable pain). Additional drug requirements during L, 6., and 12. hours were recorded. Results were compared statistically; p<0.05 was considered as significant.

Resul t s: No significant difference regarding age and vveight were detected. On the l.hour 17 patients in group l and 30 patients in-group 2 required analgesia. On the 6th hour, 20 patients in group l and 30 patient's in-group 2 required anaigesia. Except 2 patient's in-group l, ali patients needed additional analgesia during the first 12 hours.

Conclusion: The combination of opioids with nonopioids provides efficient postoperative pain management. it is shown in some studies that tenoxicam which is an analgesic anti-inflammatory drug reduces opioid consumption without causing any side effects. in this study, we found that 20-mg tenoxicam reduced meperidine requirement when used in combination with meperidine and this provided efficient analgesia
